SLGS dosage question for Zeke

I have officially decided on slgs finally. I have new syringes with .5 u markings, will start a new Lantus pen in a couple of days, and updated SS, as of today. Lately just testing preshots and nadirs with occasional randoms, but am now ready to try this dosing method. I increased him to 1.5 this morning from 1.25, his numbers are staying high at nadir, so... I will hold for 5 days. Correct? I am working until 10/31 so will be doing a curve then, so I'll actually be holding for 8 days. My question is the instructions say to start at 1 u. That isn't true if he's been on 1.25 for this long is it? He is already doing better (125) this afternoon, with the 1.5. Thanks.

You went the wrong way with the dose, if following SLGS. That 59 on the night of the 18th means he earned a 0.25 unit reduction, but going below 90. His new dose should be 1.0 unit. We determine the dose based on how low it takes the cat. With SLGS you are looking for a dose that gets you nadirs in the 90-149 range. It's likely the high numbers you are seeing are because of bouncing. So, lower the dose to 1.0 units and hold for 7 days. Unless he goes below 90 again, in which case you would immediately reduce the dose a further 0.25 units.

Ok ,I'll reduce to 1.0 u tonight. It's hard to think this way when seeing a good number after an increase, but I guess that's why we're stuck at 1.25-1.50 for so long! Thanks. Will do. I'll just ignore the higher numbers I'll probably get(?) for a while. Thanks.
