Yesterday. Caber Yr 2 Day 7

She is coming to work with me. Gave tsp of the Paws for shot time which I think is about 8%. Given the +1 she probably is safe leaving home with food out and Husband would definitely over feed but more comfortable if she is with me. So gotta pack up all her stuff and cart it in. Will Catch up when I can log in at work.

I gave reducie this morning anticipating limes today/tonight as her pattern, 10 days on a dose.
 
That's really great that you can bring Ollie with you Paula. More stuff to pack and bring but she'll be safe and you won't have to worry about her.

Ollie all settled in her play pen in supervisors office.

Rough start already today. Doors to let me in didn't work. My dad was asleep on the sofa in the lobby when I got here (not a new thing). He tumbled off. Didn't get hurt. The fire panel has an alarm on and no one knows where area 329 is. There are cryptic messages for me to do something and I have no idea what it is.
 
Ollie is looking good! It is nice to be able to take them to work. I have done it a couple times when I was worried about Scooter. I don't think the boss really appreciates it, but he can let me bring him or I will stay home! :p

She didn't eat all day except 3 freeze dried treats.

Think I'm gonna go back to the 5.5 units. I reduced anticipating limes tonight and let the depot work. But since I took her with me she is all out of sorts now. Before this morning I was actually thinking an increase might be needed. So will let the 5.5 work a bit longer.
 
Oh my goodness!! What a day you had! Doors? Alarms? Area 329??? And now Ollie is all wonky because of her trip to the office...

:facepalm::facepalm: I sure hope things settle down tomorrow :)

Sorry to just get here; I fell asleep early last night. We got our first Shingrix vaccines and ....well, they aren’t fun :p

Her labs look pretty good. Her USG is low which, before SDMA, was an early indicator of CKD. Her SDMA is “just” in the normal range and her creatinine is on the high end of normal which is not uncommon in acro cats. Her P is a little high (but lower than it has been) and I might try to focus on getting as low of P foods into her as much as possible (I have a feeling you are already doing this because her trend has been downwards). She also has 1+ protein in her urine again. Because acro cats can be prone to proteinuria (Neko had it), then you might want to ask the vet if there is any extra urine for them to do a protein:creatinine ratio just to be sure. Or you might want to do a free catch and have it run. Her microalbuminaria (MA) have been high in the past and that’s another hint that there could potentially be an increased P:C ratio. I see she is already on benazepril which is the usual treatment for proteinuria; if she is proteinuric, then you’ll probably need to talk to the cardiologist before you change the benazepril dose.

I do think the labs bear watching because it’s possible she’s trending towards CKD. USG being low is not always a sign of pending CKD because, as the day progresses and cats drink more, the urine becomes less dilute just as in humans. And hers has also been lower before. But, because acros can tend towards CKD, her SDMA is on the border, her creatinine is still towards the high end of normal (and that can also have to do with body size), plus the high MA in the past and protein in the urine, I’d just be aware that could be what’s happening. Or, because labs are just a snapshot in time, it might be nothing. Sorry to be so decisive haha.

My goal is to just bring a few things to your attention that your vet might feel is normal but which are borderline. I have had vets look at parameters independently and miss the CKD (or other) dx. Right now, I’m sure the vet would say she doesn’t have CKD but, as I said, she might be trending towards it.

Thank you Marje. CKD has been on my and vets mind for quite a while. But she always seems to be having another issue that required blood work vs kidney panel.

She drinks a ton and gets fluids so vet said that's why dilute. While earth duck changed their formula so it's higher in carbs and phos now but still acceptable though Olive won't eat it now. It's hard finding a low phos food now. But keep trying.

I thought the ratio was there or at least I figured they would do that automatically if doing kidney panel, but what do I know.

Will keep working on it. Thanks again.

The urine P:C ratio was not on there that I could find. It’s a specialized test and can be $$ so they don’t run it unless it’s asked for. And your vet probably wouldn’t run it without checking with you because of the cost. But I wouldn’t delay it considering her MA levels in the past and her acro. Better to spend a little $ and get to the bottom of it. It could be nothing....my cats both always have 1+ protein with negative MA; even though they are both young with no CKD, I’ve run UPCs on them just to be sure. There can be other reasons a cat is proteinuric. You may recall that Karen’s (Doodles) one year old cat, Leo, has idiopathic proteinuria.

I will talk with vet about doing it. Yes I know Leo has it. I do want to keep possible ckd out of the picture.

There’s not really any way to keep it out of the picture. No one has control over whether it develops. Over 50% of cats over 10 have CKD. By ensuring she gets wet food with lots of water and keeping the P low, you can potentially keep it at bay a little longer but the acro, honestly, doesn’t help.
