Help For Molly #15

Molly was pretty flat all day to day so I'd reserve judgement about an increase until we see where BG is in the AM.
Yes you can switch to TR. That would mean you wouldn't reduce dose until BG falls below 2.8 It's aggressive and it can be done while working full time as long as you can get home at lunch to test, It's entirely up to you. Have you read through the sticky for TR? It can also mean some sleepless nights on occasion but it can also be well worth it.

Sorry Adrienne. Wasn't online last night. You made the right decision to go ahead with the 0.25u. Stay with that dose unless BG is at 50 or less at pre-shot. When you test at night a couple of hours after shot, and it's the same as or lower than pre-shot BG, that can indicate it might be an active cycle so it's a good idea to set an alarm to grab another test and leave out some food for Molly to eat if needed.

I posted on the other thread. To last night and no one responded o_O. Can be a little frustrating!
Molly ate a little too much after her bedtime test, was sick she slept with me all night. I was awake the majority of the night monitoring her.

Sorry Adrienne. The board has been extremely quiet recently and weekends are always quieter. I'm hoping traffic will pick up again now that fall is coming. I'm usually online at your shot time but got waylaid last night. :blackeye: How is Molly feeling this AM?

It’s ok Linda just wasn’t sure what to do. Molly is good this morning. She ate her breakfast and is now looking out the window. I overfed her last night and that’s why she got sick. I’m a little scared if and when she does hit the 50 mark. I’m hoping it’s on the weekend.

Adrienne, the 50 still has a safety margin built in so while you need to take action if she is that low, it doesn't mean you need to panic. Those first greens can be scary but if you know how to deal with them they don't have to be. Make sure you have some medium and high carb food in house. Get some cans of gravy lovers FF and use the gravy to bring numbers up quickly if Molly hits 50. Keep a copy of the hypo instructions in your hypo kit with the higher carb foods and extra test strips and some honey or corn syrup. If you have your kit together, you won't have any problem.

Last but not least, you hold the needle and if you are uncomfortable shooting a full dose or any insulin for that matter, do what your gut tells you to do. Molly will get back to business soon enough if you decide to skip a shot or give a reduced dose.

Ok, I'm not use to those numbers and panic. I have extra strips and bought some FF gravy. Is that considered a high carb? and what would be a medium carb?
I have honey too, I'm well prepared in case Molly dips low. I wanna keep my baby safe. Do I skip when she gets that low?

Technically you can give insulin at BG readings of 2.8 (50) or above HOWEVER you work your way down to shooting at that level. You've shot as low as 3.9 (70) with a dose of 0.10 and Molly surfed along beautifully but when you can't monitor then that might not be the right decision. Lantus is great at holding numbers much better than it is at pulling them down so you can shoot lower BG and still have kitty surfing. There tends to be less drop when starting a lower BG. That doesn't mean you get complacent but once you know what Molly does, it gets easier to shoot and not worry.

You can use the higher carb foods for days when you shoot lower but want to make sure Molly doesn't drop too low. Medium carb would be between 11 and 15% and I think some of the flaked/sliced/chunk varieties of FF fall into that range. I suggest you check Dr. Pierson's list for flavours Molly might like. My fussy girl is a one flavour/texture cat so I never had the option of using different foods with her. My go to for high carb with my girl was sugar donuts! :woot:

I just saw the note on your SS about Molly being sick today and was going to ask about it? She vomited tonight? Wasn't a scarf and barf I take it. How are her bowels now? Is she eating again now?

I was wondering if she might have gone a bit lower last night with that lower pre-shot but maybe she's up because she's feeling off.
If you get up to use the facilities in the middle of the night, it wouldn't hurt to grab a test occasionally to see what she is doing later into the night. If you can't during the week, consider setting an alarm on the weekend just to get a better picture of her nighttime activity.

I came home and noticed the vomit by her litter box. She seems fine meowing for food . I think she’s a little constipated. I’ll have to give her clearlax. I’ve been slacking on that. Not sure how often to give it to her. If I get up through the night I’ll test her. She’s running around so I think she’s ok.

Running around sounds good. :)Could she possibly be trying to present a hairball? My old guy used to occasionally vomit like crazy for a couple of days and then produce a monumental hairball.... granted he's a long haired cat but shot haired cats get them too.

I always find that the change of seasons is the worst time for hairballs and my long hair was never particularly impressed with being brushed anywhere but his mane. The rest of him has always been a struggle at best.

I think dear little Molly needs a dose increase to 0.5u tonight. With TR, you can increase the dose after 6 cycle if you still aren't getting BG readings in the range you want.

That's OK. A lot of cats go up before they come back down. Molly just needs to learn that it's OK to be in lower numbers so she stays there longer so we don't want her swimming in yellow too long.

Doing 0.25u is really tough because the lines on the syringe are so close together. It's more important that you do it consistently than being 100% accurate because it's just not possible to know exactly where the quarter dose is on the syringe. The markings on the syringes don't help because the lines can be way off on some syringes. The one way to really finesse dosing is to use calipers. If you are interested in doing that, I'll find the instructions. I used them and it does make things easier once you do the preliminary work. You need digital calipers with a locking mechanism and they should be available at Cdn Tire, Lowes, Home Depot, Home Hardware etc.

I can't tell from the pic exactly where you were drawing to but if you were between the first long line and the first short line in the left side lines, you were in the right vicinity.

Ok. As long as you know what you've been doing we can sort that out. If you've been giving her .5u then up it to .75u which would be between that .5 line and the first full unit line. Maybe hold off till the weekend to increase though so you can monitor and sort it out then if there is any doubt in your mind.

Yes I think I would take her up to 0.75u and see what she does. How are her bowels? Her Asthma? Any more problems with vomiting since Tuesday?

OMG! I think it's best to skip now that she's eaten. Please check her BG 3 hours after she finishes eating. That will give us a clue as to what her pancreas is doing. Next time stall , don't feed her and retest to see if BG is coming up on it's own. :)

Yes. When doing TR, you don't reduce dose until BG drops below 50. Technically you could have shot this AM but when you get a low number like that best not to feed and retest without food to make sure BG is at least steady or on the way up before doing so until you see how Molly responds. I was hesitant to suggest that mainly because of the confusion about what dose she's been getting. Otherwise I would have told you to go ahead and give the dose. Let's see what she does tonight. Make sure to get a +2 or +3 test and if it is lower than PMPS then set an alarm to check her again a bit later.

That's fine. Just don't feed her now and retest 30 minutes after the last test. You can shoot full dose on 58 but she will need monitoring. She may bounce but then she might not. You can always give her a reduced dose for tonight to keep numbers down but not down too much. Let's see where she's at in a few minutes.
