NERVOUS-PMPS 140, +2 142 both w/AT Meter switched to Contour +5 63

I had to switch to a human meter because I ran low on the AT strips.
He has been bouncing between 60 and 65 for +3, +3.5, +4 and coming up on testing at +5 in 40 minutes. Just fed the rest of his dinner about 30 minutes ago. Hoping his numbers come up. I am freaking out as this is the lowest Orson has been.

Is it time for an increase?

Yes.....He's looking much better on the 2U than he was on the 3U but it's still not quite getting him into green nadir's

Since you're doing TR, you'd increase to 2.25U

On TR you can increase as often as every 6 cycles (3 days) if the current dose isn't getting him where you want him. You've held the 2U dose for 8 days (15 cycles)

From the TR protocol:
  • Each subsequent dose is held for a minimum of 3 days (6 consecutive cycles) unless kitty earns a reduction (See: Reducing the dose...).
  • Adjustments to dose are based on nadirs with only some consideration given to preshot numbers.
Increasing the dose:
  • Hold the dose for 3 - 5 days (6 - 10 consecutive cycles) if nadirs are less than 200 before increasing the dose by 0.25 unit.
    • if your cat is new to numbers under 200, it is recommended to hold the dose for at least 8-10 cycles before increasing.
    • when your cat starts to see nadirs under 100, hold the dose for at least 10 cycles before increasing.
  • Increase by 0.25 unit after 3 days (6 consecutive cycles) if nadirs are greater than 200, but less than 300.
  • Increase by 0.5 unit after 3 days (6 consecutive cycles) if nadirs are greater than 300.
Reducing the dose:

The TR Protocol is an aggressive method in itself. The modified version of the protocol is slightly more aggressive. Let's keep all our kitties in the Lantus, Basaglar, & Levemir ISG safe by taking reductions when appropriate.

Orson has not been less than 200's for 3-5 days? He is new to being under 200-it says to hold for 8-10 cycles? I am confused now Chris

Ok thank you - so increases are by the cycles of insulin amounts given and not BG test?

No....it's a combination of both

If the current dose has been given for 8-10 cycles and the BG isn't in the range we want them in, we increase.

Since Orson is "new to numbers under 200", we hold his dose for 8-10 cycles for now. Once his body has a little time to get used to being under 200, we may increase his dose every 6 cycles to nudge him down more into those greens we look for at nadir.
 
You have been at the 2.0u dose for 8 days/16 cycles. HOWEVER, with TR, if you skip a dose, the shot "clock" starts over. You skipped the PM dose on 7/9. Thus, you will have given the 2.0u dose for a total of 8 cycles after you shoot tonight. You would increase to 2.25u at AMPS on 7/14 or no later than 7/15 at AMPS.

Ackkk...you're right Sienne…..I've been trying to remember too many people's details.

I know what I was thinking though.....I know Edye can't get mid-cycles during the AM cycle due to work so was thinking it might be better to do the increase a little sooner so she'd have tonight and tomorrow to see how he did before having to return to work on Monday

@Edye Been keeping an eye on you tonight. I know you shot 2U but it's not on your spreadsheet.

Orson is looking good tonight. Hopefully he'll drop down into green tonight but if he doesn't, I'd take him up to 2.25 tomorrow.

I know you're planning on testing again about +5.....if I'm not here, hopefully @Bron and Sheba can check in on you.

If you keep your subject line updated (add new test results as you get them) it will help. The members who scan the forum look at the subject lines for people who might be in trouble first.

Also, if you want to "bump" your post back to the top of the forum (they can get lost in this busy forum), just make a quick comment and it'll bump your thread up to the top again.

I noted that I switched meters so people didn't think he had a wild crazy drop.
 
Good thing you didn't increase tonight!! Looks like Orson is going to be one of those cats that waits until the last minute to show what a dose can do!

Give him a teaspoon or two of his regular low carb food and continue testing every 30 minutes for awhile
