Yep, sadly, that’s what happened. Sadly, my vet of 20 yrs whom I’ve loved and found trustworthy thru many beloved pets, didn’t test for ketones—for a week of taking Principessa to her daily. Ugh.
I see you are giving subQ fluids. How much and how often?
I’m giving her 100 ml 2x day. Regardless what the vet says (taper her off), I see they make her feel better and feeling better = she eats. I did have a sense they were “flushing ketones” though I didn’t have that specific info at the time.

This is where I’m struggling a bit. When she wants to eat, as she is now, she seems ravenous the entire day.. So, it’s difficult to feed only every 12h. It seems eating is so important; her poor little body has been quite ravaged by the DKA, and seems she needs to rebuild. But when I feed her all she wants, it often drives her BG up very high. For instance, last night I had waited 12h to feed her: she ate 4 cans of food! 3 cans FF Tuna Fish in Gravy Grilled (13 carbs/can) and one can FF ATURAL White Meat Chicken & Tuna in broth (0 carbs). This am AMPS is 412. She’s eating well right now and I’ll give her 4U Lantus.

I don’t know what the infection was, but at the time she did AB orally to start, them she was hospitalized and they continued AB intravenously. We’re seeing an Internal Medicine specialist today for the first time and I’m going to ask her to recheck white count. What would be the symptoms of infection? Every 3 days so far — corresponding to appetite stim — she gets puny and stops eating. I don’t know the root cause of this and this is what really worries me! Any ideas? This is Day 3, tho, and so far she’s eating (looking at me asking for a 2nd can of food for breakfast now. Yay!!

Most days, yes. She has had no ketones for about 2 weeks now! Wow! I’m so happy to stop and realize that.
So, she had high white count 5/19 and started AB + daily fluids, liver “count” extremely high, diagnosed and hospitalized w DKA 5/24-5/26. Crashed terribly at home 5/27 w extremely high BG + Ketones again 5/28. Saw new vet who started talking how hard this was going to be (translate: putting her down; yes, will not be seeing that vet again. Got ultrasound: pancreas + organs looked normal. Liver a little bit “lit up” but not to worry, fairly unremarkable. (I’m going to ask new vet to do liver count today.) Merciful vet at EC where ultrasound was performed gave me appetite stims so I was able to get her eating. Merciful vet at EC where she was hospitalized gave me SQF + lesson later that night. What a day that was!!!
2 wks of daily fluids, learning to test glucose, giving anti-nausea + appetite stims when she stops eating. Very high BGs and a couple of lows that were very scary. She seems to not feel well when she’s in the 100’s—tho I know that’s eventually where we need to be: Any insight on that? Because I’ve seen her come dangerously close to death from too much insulin, I’m terribly afraid of giving her too much. So glad I’ve got testing down now and getting blood from her ear!!! Huge milestone!!

UPDATE From Visit w New Internal Medicine Vet Today:
All of Princi’s bloodwork was normal!!! Liver and white count being particular exciting! She gained weight to over 8 lbs up from scant 7.3 lbs!!! The vet was so informative, helpful and validating—I feel quite blessed to have found her. She’s supportive of me doing glucose testing and continuing SQF’s. She thinks Princi has turned the corner and encouraged me to let her eat—feeding her 4x day w 2 12h shoots. She advised me on her insulin for her highs. She agreed I should always have anti-nausea and appetite stims in our medicine cabinet and wrote generous scripts.
Everything I’ve learned — mostly from you all—has worked!! It’s an incredibly happy day for Principessa and I and please give yourself a huge pat on the back as well. Big hugs and purrs from us!
