Spread Sheet Instructions?

I want to set up Jack's spread sheet, but for the life of me I can't find the link to the instructions.
Can someone give that to me? Thanks!

I wasn't able to get enough blood for a BG reading yesterday, although it looked like enough blood to me. (I kept getting an error message.) I didn't try this morning. I'm going to pick up a ReliOn meter today, at Walmart. I'm hoping for more success with that one.
