12/26 MARIO, AMPS 229, 120 +6, PMPS 455

I know it's hard, but keep the dose stable for now. Mario is in a bounce, and he may be a cat that stays high for more than 6 cycles. His body is reacting to the numbers from AM on the 23rd - a sharp drop into the blues will do that. Your best friend right now is data - as much testing as you can work in and frequently posted. Eventually, he'll drop again, and you'll need numbers to get good advice on doseage after that.

Keep going. Be patient. It will get better.

ok. numbers are looking better today so far.

Not sure why your links aren’t working, but that +6 is looking awesome.

Thanks!

I was wondering when I can start lowering the dosage since looking at Mario’s spreadsheet, he had pretty decent numbers when he was in the 4-5 units.
I increased the dosage too quickly.

Please keep that spreadsheet updated with the latest numbers. They are beautiful, let’s brag about them.

Mario will tell you if this dose is too high by going below the reduction point, he’s not there yet. If you are following SLGS, that reduction point is 90, 50 if following Tight Regulation as your dosing method, So keep the 6.5 unit dose for now.

Please don’t tell me I have to see numbers below 50. Too scary for me . Yes, I’m following TR. can I look for a more conservative number (for my peace of mind)?

If it helps any, 50 is the number we use that we don’t want cats on insulin to be below. Cats not on insulin can test as low as 40. Tested my non diabetic cat in the 40’s. If 50 is too scary for you, you could always follow SLGS for a a while and use 90 as the reduction point, and switch back to TR when you are more comfortable with lower numbers. Greens are scary at first, but eventually you crave them. Those numbers are easy in the pancreas and below renal threshold, Once you learn how to feed to control the BG numbers, it’s much easier,

With an AlphaTrack meter, the dose reduction for TR is a bit higher -- 68. The AT meter is calibrated a bit differently and the instructions for both TR and SLGS are based on your using a human meter. (The strips are a whole lot less expensive for a human meter!)

I agree with Wendy. Tight Regulation is meant to be more aggressive. It has safety built into the protocol but it's not for everyone. With SLGS and using a human meter, dose reductions are at 90.

We don't recommend that you try to merge the two methods for dosing. If you follow some elements of TR and some of SLGS, the rest of us won't have a clue what you're doing and it will be hard to offer you any recommendations. However, it's perfectly fine to start with SLGS and once you're comfortable, switch over to TR.

Since you are using alphatrack 2 for your meter, your reduction point for tight regulation is 68.

ETA - Didn’t see that you were already given that info.

Ok, thanks so much. I’ll read about SLGS

This is pretty common, from what I've been seeing.

Please look at Yemala's spread sheet, back in late October. She had a lovely day or so, then suddenly jumped up above 500 and her numbers didn't come back down for awhile. The reds and blacks are scary, but this is how the whole thing works.

Mario's body is used to higher numbers. He had a really sharp drop today, about 100 pts. When that happens, sometimes the liver panics. The lower numbers feel dangerous, so the body dumps glucose into the blood and you get a bounce.

Be patient. Look at the spreadsheets for other kitties, particularly those who have been on the board for awhile. Mario has been on Lantus for a bit, but not with a consistent dose and without regular tests. You got a nice blue number, and he bounced. He may be high for another 6 cycles, but this is NOT the time to alter the dose again. Not unless he "earns" a reduction. If you are doing the SLGS method, which would be a good choice if you can't get a lot of data in a day, you want to look for a drop into greens before you reduce the dose. Please look up the sticky for the protocol for SLGS, and just keep asking advice.

Right now, patience. Get data, hold the dose, ask for support when you need it. This gets better.

Ok, I will try to relax. I won’t increase the dosage anymore. I won’t make that mistake again. Also, I will read about the SLGS, however I would prefer to stick to TR since I’ve finally figured it out.

Mario had a good day, although Mario doesn't know it. So he went back to where he is comfortable (red number). It is called a bounce. In time he will become more comfortable in lower numbers that last longer. Follow the protocol you are comfortable with. Keep posting and asking questions. Patience.

So, if he is going to keep bouncing for a while, isn’t it good to lower the dosage to a more reasonable amount, like 4 or 5 units for example? Maybe he would get the same numbers? Is insulin bad for the liver or other organs? Isn’t the lower the amount of insulin the better for Mario?

No don't lower. Actually the higher numbers over an extended period do more damage to kidneys and renal system. Pancreas heals when it's in normal numbers. It takes time to heal.

Only by seeing lower numbers will Mario’s body become used to lower numbers. It’s because he has become used to being in higher numbers that he bounces. Lowering the dose would just keep him in higher numbers. If you are following TR, it would in fact be time for an increase to 7 units. Doses giving nadirs below 200 are held 6-10 cycles. Tonight is cycle 10. I hope you will be able to get a before bed test in tonight.
