Spike very nauseous starting last night, did not want to eat dinner--syringed all, gave regular shot. (2U).
This morning, still nauseous, syringed breakfast. Readings in title. He has chronic pancreatitis and chronic allergic rhinitis/sinusitis, along with asthma. His rhinitis is bothering him, too. Have been treating him for pancreatitis flare for about 2 weeks. Seemed okay until last night. Wanted out today to eat grass. Tested him last at +7 today, then let him sleep after lunch and extra ondansetron.

Tested at +11, 46!! Tried to get him to eat his HC favorite food, but because he is nauseous and congested, he didn't want to eat much. Gave him HC temptation treats, LC pate and some karo syrup (maybe half tsp) He did NOT want to eat, but syringed into him careful not to overwhelm him. He is angry and ill. Got his numbers back up. No shot tonight.

We will continue to test him every half hour to hour all evening. Once his reading was over 100, put him in steam box to help break up the congestion. What can I expect now? I'm sure I will drop him by .25U to 1.75 from 2U tomorrow. He is ill. Bad pancreatitis flare/sinusitis flare--both at once. This poor cat....can anybody offer any help? I need MC and HC pates. I would need to syringe. Are there any out there? I will look at food chart once I can spare the time.
 
You can water to any HC food and use a blender to make it suitable for syringing, I used an immulsion stick blender. First Mate, if available near you, makes pates that are HC. Or just add karo to reg7kar food you are syringing. Since he is on he is on his way up anyway, you can probably stick with LC anyway.

I would reduce his insulin, as you have suggested.

How many times are you giving ondansetron and how much does he weight? If is often underdosed.
 
Yes he earned a reduction, great job and sorry no one responded, I’m just seeing this, u did great getting his numbers up, hope he feels better

I have been giving him 2 mg ondansetron BID, but today adding an additional 2 mg at lunchtime for a total of 6 mg today. He is 16 pounds. What is the suggested dose?

I also made up some slippery elm bark, but have not given him any yet. Made it while he was napping today. Then tonight, he had other plans.:nailbiting:

I will try to stick blender, or can try a little "bullet" blender. I guess I would have to blend down the whole can. I will play around with it. thanks.

His new dose in the morning will be 1.75u. You can shoot at your normal time in the morning or, if you wish to pick a new time, now is the chance to do it.

I see you gave him HC treats to get the numbers up after the lows at +11. Just a note for future reference: it was fine to do that tonight if you did not have the intention to shoot since he’s not feeling well. However, if, in the future he gives you lows near a preshot, you would want to follow the advice in Dealing with Low Preshots. We don’t give HC food to get the numbers up to shoot because it gives us a false sense of security and the numbers could come right back down.

So, if they are in the 40s, we retest in 15-20 mins and if the number hasn’t risen, we give a small amount of LC food and test again in another 20-30 mins to see if that brings the number up above 50 where we can shoot. By +11, the insulin should normally be waning (unless the kitty is clearing a bounce) and so even LC food can bring the BG up.

I do understand that for tonight, you probably wouldn’t have wanted to shoot anyway due to his issues so it was ok to feed HC treats to get the BG up.

On another topic, his SS isn’t scrolling correctly so that the first few lines are frozen and move with the SS (e.g. the line with the + hours in it). If you’d like me to fix that for you, I can. Just shoot me a PM by clicking on “marje and gracie” to the left and “start a conversation”.

I hope he feels better soon (he’s very very cute) and you both get some rest.

You can do 0.5 mg/kg, three times a day up to 1 mg/kg. I'm presuming you are giving pills vs injections. At 16 lbs, that means 7.2 kg, so you could go 3-7 mg each time. That is, if you think nausea is the issue. Sounds like it, poor boy.

Please let me clarify. I had every intention to shoot tonight; he rarely feels good, but I have to juggle all his illnesses at once. Because he tested out at 46, I figured I had better get him up to a safe number before I lost him. It's easy to sit back now after the fact and say, he wouldn't have....but when I was going thru it, I sure didn't have my crystal ball plugged in. He has never dropped that low and I never intend nor want him ever, ever to drop that low again. So everything I did and fed him was to avert a hypoglycemic event. Or maybe I should have waited to see if he dropped more? If he won't eat HC food because he is nauseous and congested, I feed him what he will eat. Whatever I can to raise him.

I've read all the stickies about waiting and re-testing in 30 mins, etc., etc. and I have done that, but as I said above, this was not one of those instances.

No, I never get rest anymore. Trying to treat this illness around his other issues constantly, is all I do.

Thank you. When he is really nauseous, like today (lip smacking, grass eating, not eating) I will increase his dose. I appreciate the info.
 
Rusty had chronic congestion. For a while we used Standard Process Allerplex: https://www.standardprocess.com/Products/Standard-Process/Allerplex#.XB2WwPx7mUI
It seemed to make his breathing easier. I gave 1/2 capsule a day mixed into all-meat babyfood. Allerplex has a strong taste, so you wouldn't want to put it into Spike's regular food. We gave Rusty his dose in the middle of the afternoon and called it his "lung treat".
After a couple of years he tired of it, so I stopped giving it. Maybe I should have continued. Sometimes, when he was having a flare, he would sneeze 17 or 18 times consecutively--great gobs of yellow snot. His breathing was sometimes so loud you could hear him a room away! He was such a strong cat. He never let his "condition" interfere with his joy in life. He didn't have asthma.

Thank you so much! We appreciate your good wishes! This sounds exactly like what Spike has. When Cornell University scoped him they determined that the little bones in his right nostril had been destroyed due to a prior infection that didn't completely resolve. Could have been from when he was a kitten, they didn't know. But that he will always be prone to congestion, sneezing, gobby snot, etc. I honestly wish he would just "sneeze" it out. He would feel so much better. But most times, we can put him in his steam box to help break up the mucous. No cure, just care to lessen the symptoms. Thank you for the suggestion. I will give it a try!

His asthma is pretty much under control. I have him on the lowest dose of Flovent right now and he hasn't coughed in a very long time!

We suspected that Rusty may have been injured--perhaps kicked in the face or hit by a car--during his time on the streets. When he was found wandering the streets in Plymouth MA in 2008 he had many broken teeth that had to be extracted. He was also found to be diabetic at that time. We adopted him in 2010 and he had the breathing/congestion then. We had him tested for asthma, etc., but he was negative.

Nothing was offered as criticism of what you did. I just wanted to be sure that, going forward, when faced with a similar situation, if you felt the need to give HC treats, you would need to wait a couple hours before shooting to ensure you weren’t shooting a food spike.
