DAY 2 on Lantus.

Working on getting Booger back on a 8/8 schedule. Was on 11/11 as of yesterday. Today was a 10:30/10:30 day. Eating small meals through out the day. Currently getting 2 units of Lantus. Was previously on Vetsulin 1.5 units a day. I questioned whether or not to start him on Lantus with 2 units as prescribed by the vet, but I didn't follow my gut instinct. So here we are day 2, I know I have to hold the 2 units for at least a week. He is not curving at all and I am not sure if that is a good or bad thing at this point. I am however glad to see that he is not going from a really high number to number 100+ lower very quickly as he had sometimes before. I have read all I can read on the stickies. My mind feels like mush. lol. I am not sure if his number are staying in the 300-400's because he is not getting enough insulin or of its because he is bouncing. I guess we will see.

For a while, I stopped documenting his number as he was on a good dose (1.5) even though they are all in the meter. I wish I had never done that but I am going to try to fill in the blanks the best I can. Any suggestions for this newbie on where to go from here?
 
First suggestion, put the preshot number in the AMPS and PMPS cells of the spreadsheet. If you have to delay, put something like 516@+14 so we know you delayed two hours. Some people have a time column, which would give the same info. Then the following tests are out in their regular spots after the shot.

Second, the +2 test is a good one to get for a lot of kitties on Lantus. Note my girl, she like to be different. But for most, it can be an indicator of how the cycle might go. If the +2 is the same as the preshot, it's likely to be a normal Lantus cycle with some downward movement. A larger drop to +2 means an active cycle that will need extra monitoring. A higher +2 means little action and a good chance to sleep at night. Last night was an example where you didn't need those later tests, unless you were just data gathering. Which can be a good thing when you are new to Lantus.

I would be extra vigilant because you started on a higher dose than we normally suggest. Time will tell. You would hold a week, unless he earns a reduction be giving you a number below the reduction point. Are you following one of the dosing methods used here? That is, Tight Regulation or Start Low Go Slow? Just wondering why the "do not copy dosing" in your signature. I have it in mine because my girl had two high dose conditions. The dosing methods here tell you what the reduction point is.
 
Now, we wait...patience, give it a few cycles more to see what Lantus is going to do...no one likes seeing high numbers, but you need to put these on..a nice pair of comfy patience pants. Well, those are my thoughts, anyway. Don't forget to check before bed test, at least tonight.

Thank you for the suggestions. I was not aware of the methods on here until later in the day yesterday. I did read that they needed extra monitoring at least the first 3 days when starting Lantus, so yeah I am data gathering. I have "do not copy dosing" in my signature, but I dont want some to come along and think they should give their cat 2 units of insulin when their cat reads at 400 something. I am very confused with all the jargin. I read abour reduction points and all that but reading it just now, my mind draws a blank. I will have to go back an reread some stuff I guess.

Thanks Christie, yeah I know I have to wait. I am trying my best. So tired of him being high like all the time but I am glad we finally made the switch over to Lantus and I just know its going to do good things for him.
 
You'll get there, don't get discouraged :bighug:, I try and watch how my kitty is feeling too, sometimes it isn't just the numbers that matter...is he playful, cuddly, purry, eating well. Like tonight, Mav decided to stick around 360 on AT2...but he's current running around upstairs, probably chasing his little spongy ball. I'm ok with that, tomorrow's another day.
 
Well it looks like I may be following the TR protocol with out even knowing it. lol He is on a 12/12 schedule, and he weights 9.8lbs or 4.5kg and he is getting 2units which is .25 units per kg. Technically his ideal weight is 10-11lbs so he may technically need more but I am holding the 2 units for one more day. So it says on the TR protocol:
  • Hold the initial starting dose for 5 - 7 days (10 - 14 consecutive cycles) unless the numbers tell you otherwise. Kitties experiencing high flat curves or prone to ketones may want to increase the starting dose after 3 days (6 consecutive cycles)
So would Booger's numbers from the last 3 cycles constitute as a high flat curve? Just trying to get prepared if the number stay pretty consistant the next 3 cycles. If so, I would increase him by .25 on the 7th cycle and hold that for at least 3 days?
