Hi everyone,
I had Dr. McCue call me this morning because a full 7 days of the thyroid increase (from .25 twice a day to .35 twice a day) was yesterday. I just called off because I am just too overwhelmed with all of this to try and work. He didnt want to eat this morning and has been mean. I am just going to type out what I have written down from his schedule -
630am - 1/2 can of food with 1/2 Gaba, Lev and Pred hormone liquid meds. Ate good. (Night before - last full Gaba @ 830pm)
7am Desmopressin shot. Mean, growled.
930am - 1/2 can of food with 1/2 pill probiotic
130pm - 1/2 can of food with 1/2 Gaba
630pm - 1/2 can of food with Lev and Pred, and desmopressin shot
830pm - 1/2 can with full Gaba for nighttime, only ate half. Gave Cerenia (1/4 pill)
Found a better poop too.

Today
7am - 1/2 can of food with 1/2 Gaba, Lev, and Pred. Since he didnt eat the full meal with the Gaba last night, he was not hungry this morning and did not eat. He does not eat unless he is on the Gabapentin.
By 9am I gave him fresh food (and meds) and tried again. He ate about half. He is nauseous I think like Wendy was saying - because he sniffs it and I can tell he wants it but doesn't eat it. Also gave Desmopressin shot. He was acting ok this morning really - on DH's lap and hasnt done that in a long time and watching the birds. But wont eat and also got mean later in the morning, hissing and both of us. I hate it. This is not him at all. This is when I had Dr. McCue call me.

I said I can't do this Gabapentin anymore, I cannot figure out a schedule that will help him eat but also not totally zonk him out. Plus I feel like I am constantly worrying about when he needs his next meds, and I have to go into work next week one day and will be busy all day Friday taking photos for my cousins wedding (got roped into it, really dont want to), so I dont know what I'm going to do while I'm not here.
He asked how he was acting, like going to favorite spots and not totally lethargic, and I said its hard to tell while he is on the Gaba since it makes him act sleepy, but these last few days on the 1/2 Gaba he seems to be doing better, going to favorite spots and watching the birds, BUT he also gets mean and hisses sometimes. It is so hard to figure out. He said going to favorite spots is a very good sign, like you all were saying too. So I am happy about that. I do agree that he is doing well overall, but I need to figure out a better plan to get him to eat / feel better if he is nauseous, and he is still acting mean sometimes too, and just not himself at all. He also asked about his weight and if he was grooming himself and if his coat looks flaky/dandruff and I said I have seen him grooming himself, and he has no dandruff. His weight has been up and down. The day I brought him into the vet because he wasnt feeling good, he was 16, then only a few days later at the vet he was 15.5. But at home the other day I weighed him and he was 16.6.... so Dr. McCue just said its good he isnt losing weight, that is what we would need to be concerned about. I also brought up the Ondansetron that some suggested in replacement of the Cerenia, but he said he would like to try upping the Cerenia dose first, to 1/2 pill every day. Which I was suggested here too so hopefully that helps. He also said I should try Bupe in replacement of the Gaba for now and see how he does on it.... So I am picking up a shot of Bupe from the vet to start him off. Dr. McCue said it is a liquid 2-3 times a day but Dr. Lindsay said I can start off with a shot to get him through the weekend, which sounded good to me because I have a vendor show now tomorrow so I will be gone all day..... When I called Dr. Lindsay to get the Bupe shot she said I can also do Gabapentin in a compounded liquid form from a local small pharmacy, which she said should be a lot cheaper than the Bupe. Oh yes also - Dr. McCue said to up his Pred. dose to .5 twice a day, up from .25 a day.... he said steriods should help with pain too. His cortisol level was low, but he wanted to try one hormone increase at a time before. But now since its been a week, he decided to up it. He said I do not have to stree him out by getting more bloodwork right now, which was SO good to hear. So we are going to try the Bupe and increase in pred. for now and see how he does in one week.

I dont even know if I have covered everything it all may be convoluted, sorry if it is. I had a full on breakdown this morning and was crying on the phone with Dr. McCue. I hope I can get this all figured out. (its 11am and he has finally decided to go eat, thank god so I dont have to throw more hormone meds away)
 
So at the vet today I will be picking up the one time Bupe shot and 4 more Cerenia pills. I need to decide what else I want to buy to have on hand for next week.... My vet pretty much said the liquid Bupe will be more expensive and the shot is every three days as opposed to liquid 3-4 times a day, so she looked up a cost for a bottle of that and its $140 which would last six months. I better not need to give him shots for six months..... but I guess thats where we are at. The compounded Gaba is $100 and should last 1.5 months so I guess the Bupe is cheaper anyway.

Forgot to mention the Desmopressin. He should have been off of it by now..... but last time we tried he clearly needed it because he drank a lot of water. S he said we can try again, with decreasing it to .5 unit twice a day.
 
I remember someone said AlliVet for the Cerenia, I need to call them today because the pills are very expensive. I wish I knew if the Bupe was going to work so I can get a bottle online too, but since I dont know I guess I should pay for the one time shots for now, see how he does, and then come Monday if he does well I will buy another shot from the vet and then buy online if needed / if its cheaper.

Another side note - like there werent enough - I did order those empty capsules Lizzie suggested, they should be arriving any day now. So maybe i can use that for the Cerenia now that hes 1/2 pill a day.
 
I just looked at the Cerenia on AliVet and it also confirms what some were saying in here before, that it is for dogs not cats, and its for vomiting. Ughhh. Dr. McCue said Cerenia is highly effective though and he wants me to try 1/2 pill before going to Ondansetron. Maybe I should have insisted....
 
Cerenia can be used for cats. It was originally developed for car sickness in dogs and they used with cats. It helps with nausea, more than vomiting. It also has some anti-inflammatory properties. The manufacturer's website describes its use for vomiting in cats.

Frosty gets 1/2 cerenia pill per day and since he started it he hasn’t had any vomiting. I am giving it to him daily until he gets back to baseline and then discuss with vet. I know it was expensive but i also picked up other stuff but it was $50.
 
First, you need to take a deep breath! You're so close to the situation that you aren't able to see that there were a lot of positive things in your first post above. Celebrate the victories! He's grooming himself, going to his favorite spots, sitting on laps, and eating well at least part of the time. That's a lot to celebrate!

Cerenia is great and works just fine on cats. Grandpa growls and gets mean, too, when we give fluids or sometimes now when we do something minor that he doesn't like. He growled at a vet who tried to touch him while he ate. I don't blame him. I'm grouchy, too, when someone messes with me. If I can be a bad patient, I figure he's allowed as well.

We have to just ride the waves with this process to an extent. Some ideas will work, some won't. As long as there's breath, there's hope. We just try the next thing in front of us. But don't forget to celebrate the wins. Shmee is there with you, that's worth a big celebration. Hugs!

I echo the "deep breath". I hear kitten steps of improvement. Lapping is huge! Shmee is headed in the right direction. :bighug::bighug:

I got my buprenorphine compounded chicken flavour liquid in a vial, three months supply cost about $100 (in Canada) and was WAAAY cheaper than the prefilled syringes you get at the vet. And much easier to tweak the dose if need be. I gave it twice a day right after the main meal. You want food in the tummy. Some people find it better to do smaller doses every 8 hours.

Good luck with the new/revised med schedule.
 
@Beck and Grandpa had me in tears yet again... thank you everyone, it is so hard to see good things when he is just doesn't even seem like my old cat, and he is in so much pain (or just mad) that we can't even touch him or he will get mad.

I also forgot I wanted to tell @Becky & Baby Girl Kitty that her story about Baby's meow coming back after a couple of months has really stuck with me. I keep thinking I hope he is the same because I miss it so bad.

DH helped me hold him down to give him the (large) Bupe shot at 2:30. Shmee was very upset but we were a good team. I really do not want to do that every three days. We will see...

It's now 4:15 now, he has eaten maybe 1 can total today. I hope the Bupe kicks in soon and he is able to eat. I had food out but I just put it up and I think I am going to try and feed him at 6:30 since he needs his hormone meds again about that time.

I am happy to get out of the house tomorrow. The vendor show is for my new Disney travel agent job, I really hope the Bupe helps so I am able to still go. I am going to a country market about an hour away from me. It seems like it's going to be slow but its only $5 for a spot so I figured it would be a good excuse to get me out of the house and get some air. I figured if I can get my business card out there and relax a little bit it would be a good day. I have to leave by 7am so we will see how his appetite is. This is my first vendor show so I have a lot to do tonight to get ready. I will check back later. I hope everyone has a nice relaxing weekend, and @Ana & Frosty @Squalliesmom @Beck and Grandpa I will be thinking about you :bighug:

May be worth looking at the liquid oral version Wendy suggested in her post above. When George was in a lot of pain with his mouth liquid pain meds were by far the easiest way for me to get him some pain relief.

Hope the bupe helps and Shmee continues to improve, as others have said try to focus on the baby steps he's taking, the grooming, lap time are all positive indicators. I'm sure as he continues to improve you will continue to see improvement in his day to day behaviour.

People ask me how I can stand to rescue seniors when I know they will not be here long and there will be pain and heartache. Sure, that sucks. But getting old, despite the pain, is a privilege. I just turned 50 and my knees scream at me. I hate it, but it beats not walking. My mom has had some strokes and has lost a bunch of weight. I hate it, but I am so lucky to still have her. Our bodies change as we get older. Our older cats are not the same as they once were. They have more health issues and regrettably some pain. But they are alive and they have the privilege of growing old. So many animals (and people) don't get that chance. Of course I don't want to see a pet in pain, but I don't think Shmee getting old needs to make you sad.

I myself would rather give liquid oral meds vs injection unless med doesn't come in liquid. I didn't know injectable bupre was good for 3 days though.

Baby steps of feeling better, I also see being mean and hissy a good sign. He's up to fighting back. Once all the hormones are leveled off, I would think he will be settled down.

Maybe not giving the meds all at once would help.

We are ALL absolutely LOVING the new place! Both Caleb and I are in better moods overall and I've never seen either of our cats so happy or active. They're loving the extra room to run around (and they LOVE the stairs!). The only bane of our existence right now is still having to get things done over at the apartment. Once that's done, I know things will be just that much better!

I really hope you are able to figure the meds out for sweet Shmee. Please remember that you are an amazing mom to that handsome man and everything will level out eventually. Shmee is a fighter and he'll continue to improve as his levels even out. Sending healing vines for Shmee and and comfort vines for you. :bighug::bighug::bighug:

How long does Bupe start to take affect? I really hope he eats more, I am so worried about tomorrow because I just went and spend like $100 to make up my table!

I also wish I would have talked to the vet about getting a prescription for the bupe liquid, because I guess he will have to get another shot on Monday now that I didn't order meds.... ot buy them from the vet for more $. We will see... I wish there was more time in the day.

I am terrified of giving him the Cerenia…. I should really do it before I leave for Walmart.... ugh he wont even look at me and when I touch him he hisses. All I do is give him shots and shove pills down his throat.. he really hates me right now
 
Amanda, you're doing a wonderful job with Shmee! Sounds like you've got a load there ...but you're capable and strong and talented! Hang in there and keep doing the best you can...he'll settle soon...just takes time and patience...

Amanda see if you can grind the Cerenia and put it in some baby food - about a tsp. My little guy always got his Cerenia like this. The reason you read it’s for dogs is because they always do more studies for dogs as opposed to cats. I’ve had three separate vets tell me how good it is for the with the addition of some of the anti-inflammatory benefits. It really worked for Merlin and he had no ill effects. Hang in here; you are fighting the good fight and getting out for the day tomorrow is just what you need. Just a little space to breathe. Shmee is actually starting to remember he’s a cat. Baby steps Amanda. You are his world and I know he is yours.

I know the feeling. Frosty is too weak to hiss or bite me now, but he looks at me nervously, and won’t even get up to greet me. When I reviewed the videos from our Nest cameras, he is SO EXCITED to see our pet sitter, he greets her and is so sweet to her. Like, UGH! Doesn’t he know I’m just trying to HELP him?! That’s why I gave up trying to force him to take his meds ( for now). But honestly, I’m so happy he is alive I can’t even complain. I don’t know how much more he will recover or how long it’s gonna last, so i just have to cherish every moment. Hang in there!!

Just FYI I get the bupe compounded into a gel I rub into her ear. Just a thought. I couldn't get the oral in her without taking a bath from her spitting it out. Or shooting it out the otherside, or missing the pocket in her cheek. The gel has been a Godsend. Big hugs, he really is doing well for what he's been thru. :bighug::bighug::bighug:

I am posting on this thread because I do not have much time, I am at the vendor show because Shmee was doing so well last night and this morning. Wow that a different cat that just one day ago. I am nervous to try and figure out our future, but he is doing better and I can tell. Coming around us, was sitting next to me watching me paint some things last night for the show today. And DH has been texting me saying he is in a good mood. This morning he did NOT like me when I gave him the Desmopressin shot. Ugh... just still so scared of me. I feel terrible... but I know it is ultimately for the best. Taking it one day at a time.

I am nervous to try and figure out our future, but he is doing better and I can tell. Coming around us, was sitting next to me watching me paint some things last night for the show today. The main thing I like about your reports Amanda - you are 100% tuned into Shmee. With that level of dedication I have full confidence that you and Shmee will get thru this recovery period. Some cat parents are not tuned in, and they count on their vet for any evaluation. That is the opposite of you since you and Shmee are pair bonded.:bighug::bighug::bighug:

Your Disney booth looks great. I hope you get some interested customers. You sure can't beat the booth price!

Sorry I didn't respond yesterday. Besides Leo's SRT anniversary post, really busy at work. Theresa asks me about Shmee everyday. So I hope you keep posting daily updates.
 
Luci only hissed/growled at me a few times when I was testing/poking her ... just every now and then...not sure why??? Try to spend some time (like you did yesterday) with him where there's nothing 'medicinal' involved..just lovin and treats...soon hopefully he'll make 'that' association and not always the bad stuff. I have a civie who's staying in the garage most of the time because she was attacked by the neighborhood ferals - and at first she was so sciterish she wouldn't come out - for anything! She was terrified...and over the past few weeks I've gotten her to come out - because it's all about the food - I bought her some special treats (Temptations) and some various kinds of cat foods - now when I go out there I talk softly and she always comes around - and then she gets put into her carrier to relax and feel safe while we open up the garage for various things - and she always, always gets treats when I pick her up...so now she thinks of me just like Luci does 'the ole feedback'...oh, it's mama...must be food coming...ah...the cat brain! She's looking a lot better and isn't so terrified...let her out on the back deck yesterday while I could supervise...and then she went back into the garage on her own...must be some real baddies out there in her back yard... Hope Shmee has turned the corner for you and that you do well at the show today! You look so cute in your 'ears'!! :bighug::bighug:

Remember Amanda, it can take people a really long time to recover too.

I had hip surgery a few years ago, and was having pain for a year afterwards. I couldn't run or exercise until a full year passed. I even got injections into the joint and took steroids for a while. Then I finally felt better, and 6 months later I hurt my back (I'm falling apart, LOL). And AGAIN it took me a year to recover from that (sciatica. I couldn't run OR do yoga!) Now I am running and yoga-ing with just minimal pain/discomfort occasionally. Sometimes we have to adjust and just live with whatever remaining discomfort we have. It takes time. We are no longer teenagers and neither is Shmee, sometimes it takes us longer than we would like to fully recover form something.

I also had a patient a few weeks ago call me FREAKING OUT that 2 days after his surgery he as having severe pain. Pain meds weren't helping, and he has had other surgeries before, it was never like this, so he felt something was definitely wrong. I spent 45 minutes on the phone reassuring him, discuss elevation/icing/adding over the counter pain medication, etc, and told him to call me back in 2 days to tell me his progress. He never called. We called him on Monday to check on him - he said he was feeling much better. Not every situation and recovery is the same.

We just have to be patient. Surgery IS a very stressful thing to go through (like someone here said, it's an ASSAULT on the body), and it can take a lot more time than we think to get better. Plus, we often say "this isn't brain surgery", but in Shmee's case, IT IS! :) So let's give him a break. I know how much it sucks to give them meds - but I do think cats tend to forgive. I hope that soon he will be taking the meds he needs with his food, and will no longer need pain meds/desmopression/cerena, etc. Just the amount of meds he's taking is overwhelming, much like Frosty's. I hope we can be down to the essentials soon. You will get there. He is a live and well, let's celebrate that. :bighug::bighug::bighug:
 
@Ana & Frosty thank you :(:bighug:. I know it doesn’t seem like it but I’m trying my best to think that way :/ I think I have Dr. McCue’s words ringing in my ear a lot. He said a while ago before the surgery that “he would feel better than he did after his dental procedure”. I don’t even remember the exact conversation but I’m sure I must have misunderstood the context somehow. I knew it would be a challenge but never would’ve guessed pancreatitis on top of it and him being in such pain.

Awh haha, that was a good way to go about it! I’m glad it’s been working and she’s not as scared. I give Shmee treats after every poke or prod too.


Update - Steve said he was in a good mood but was also doing hard headbumps and a little bitey. He’s been very hungry. At 5pm I noticed his eyes were totally dilated, he seems a little mad. Still dilated at 5:30. He jumped on the coffee table for food three times which he never has done before, so I fed him a little early at six. I was thinking this could possibly mean the switch from .25 to .5 twice a day of the Pred was too much so I texted Dr. McCue and asked if those behaviors were ok for now, or if I should decrease the Pred. at all. He said those all sound like Bupe effects, so maybe he should get less of a dose next time. I have no idea how much Dr. Lindsay even gave me in that syringe so I’ll have to figure that out too now. I called Diamondback on my way home today and asked about the other Bupe forms. They said they offer the ear gel as well as a treat. I asked if I can buy a few to try and he said no but they are made from “Pounce Chewy Treats”, so I can go buy those and see if he likes them. Right now he is so extremely hungry I think he would eat anything though. I will probably end up going with the gel. @Wendy&Neko the bupe liquid can’t go in their food, it has to go after they eat the meal?

Got about 2.5 - 3 hours sleep last night, my headache meds with caffeine wore add about 1:30 today and I’ve been dead tired since - going to try and sleep until his 9:30 meal. :bighug: What a rollercoaster.

It’s tough to make that kind of conclusion, because as we know, everyone (humans, animals) tolerate pain differently. Also, some animals tolerate STRESS better than others. If he really does have pancreatitis, then all this pain and nausea has nothing to do with the actual brain surgery. It’s just a nasty side effect. I’m glad to hear that he’s hungry and eating. As we know, that’s half the battle a lot of the time.

Bupe liquid has to be given submucously, in English, squirt it into the cheek pocket or under the tongue. I did the cheek pocket. Big eyes are definitely a sign of a bit much bupe.

It's possible that Shmee did get pancreatitis from the stress of his hospital stay. I know Amy hated taking Trixie in to vetty cause it almost always triggered a flare. Shmee had a much longer vetty stay.

Kitty steps are turning into adult cat sized steps of improvement. :cool:
 
I hope you can get some more sleep. That is the hardest. As long as Shmee is eating and eliminating well, you should probably get some good sleep. Maybe the cool fall weather will help!
