Niacinamide diabetes and kidney failure

Hi everyone,

Pudden has been on a phosphorus binder for her kidney failure for the past two weeks and has developed severe constipation. I am taking her in today to be cleaned out.

She has suffered with severe constipation for a long time and with the phosphorus binder I am finding even very high doses of miralax along with pumpkin isn't working.

I need to find an alternative. I was told on the tanya kidney support group to look into niacinamide which is b3 vitamin. It works by blocking the absorption of phosphorus rather than binding with it. This means that constipation is not a side effect.

The issue is that I am reading it can raise blood glucose. Does anyone on this board have experience with giving high doses of niacinamide to their diabetic cat? I am considering using it any way and then raising her insulin if it's an issue.

Another problem I am having is the vet is taking their blessed time in sending over the script for her prozinc. I ordered it from vet rx direct on Tuesday night and the cut off for shipping is 3 today. After that it won't be shipped out until Monday. I have called and complained and am fuming mad at their negligence. I might have to ask today for the vetsulin which is the only insulin they carry. I know it's very harsh and I hate to give it to her. Sigh.

Will Vet RX Direct not call the vet to get the ok? 1800PetMeds did that for me the first time I ordered from them. It was within an hour of their shipping cut off, and they called and got the authorization they needed to make sure they could ship that day.

Try ordering the ProZinc from chewy. It's $96 and arrived in 2 days, in a cooler with still frozen ice packs. Free shipping
 
Thank you guys! I was finally able to get the script sent over today and the insulin was shipped. It will be arriving tomorrow:)
 
Her phosphorus was over 20 at diagnosis so the vet prescribed 1 teaspoon. She went down to 13 in two weeks. I never gave her the whole teaspoon as I felt 1200mg for a 6lb cat was way too much. I only ever gave 3/4.She is now on sucralfate for gi bleeding which is also very constipating so I lowered the aluminum hydroxide to 1/2 a teaspoon yesterday. Because her phosphorus is over 10 she should get more, but constipation is also very dangerous as far as toxin build up and possible blockages. I'm hoping the niacinamide will be a good buffer for lowering the dose and her phosphorus will continue to go down.

She was getting 1/3 a teaspoon of miralax twice a day when she became constipated. Now I upped it to 1/2 twice a day. This has caused diarrhea, but I tend to think that is better than constipation since I can just up her fluids to prevent dehydration. She also gets pumpkin.
