? 8/2 Pearl dosage advice

We had a few nice blues yesterday and AMPS 258 is discouraging.

Is it time for our dose increase or should I gather more data? For a more experienced eye there may be a clear answer here, but I'm not sure. I'm off today so I'm able to test or monitor as you suggest.
 
well, I think you decided to do the increase since you just went up to 1.25u this morning.
hold this as your new dose for now.... give the depot time to adjust.

I'm sure you've already heard this, but Lantus is a depot drug. :) It needs time to build up in Pearl's system. Two important things I learned about Lantus: Timing and consistency! Try to be sure to give Pearl's insulin at the same time of day, every day, 12 hours apart. #2 Holding the dose - very important! Here's what it says in the Stickie:
  • Hold the initial starting dose for 5 - 7 days (10 - 14 consecutive cycles) unless the numbers tell you otherwise. Kitties experiencing high flat curves or prone to ketones may want to increase the starting dose after 3 days (6 consecutive cycles).
  • Each subsequent dose is held for a minimum of 3 days (6 consecutive cycles) unless kitty earns a reduction (See: Reducing the dose...).
  • Adjustments to dose are based on nadirs with only some consideration given to preshot numbers.
I know it's easy to forget or get confused when you're new at this - it's info overload! So, please try to just do these two things - and of course continue with your testing...be patient...this takes time... :D

Lots of experts here...plenty of support and help. Keep asking questions! You may want to post a '?' when you feel you've reached the 5-7 or 3 day dose time frame so that some of the experts can suggest to you when to change your dosing. I've actually posted the very same question this evening...

I held the intial dose (with the one increase) for 7 days, so I'm confused as to why I keep getting sent the TR protocol. I mean no offense, but I am honestly and truly confused.
Bullet point #1 says to hold the dose for 5-7 consecutive days unless the numbers say otherwise... Even if you count the singular 0.25u increase, that was still done on day 6 after 10 consecutive cycles. So check...bullet #1 followed.
I plan on holding the 1.25u dose for at least 3 days per bullet #2.
I keep getting reactions that imply I never should have increased it and that I should have stayed at 1u. How am I interpreting the TR protocol incorrectly?

you did it right.
Especially if your goal is tight regulation or if you luck out, remission. It appears you caught it quickly, but if steroids are necessary for other conditions, you will have recurring lapses. You just continue treating the diabetes as needed.

Thank you! We are treating the other conditions right now with her limited ingredient diet, hopefully that will help if not, treating with cyclosporine which is not known to affect blood glucose in the way pred does. Fingers crossed and thank you again for clarifying! :)
