Princess - Finally saw some green and now in a bounce

I hate to say but I have been hanging in two different online diabetes resource places, I know this can cause some flak at times. but I really would like to get more opinions what I should be looking for and where to head.

I finally saw some greens yesterday, but it was late in the day +8 to PMPS.

So I reduced dosage from 2.25u to 2.0 at PMPS and went back to 2.25u at AMPS.

I was told by someone without a ton of experience the next bounce should not be as bad, and the bounces should get better and the 2.25u dosage looks like a good plan to continue right now.

What are the thoughts on this?
 
Hi Bryan! Have you posted on the Lantus forum before? If so, can you add a link to your last post? We do this every post so we can go back and read any background. Also add on the question mark to your title (thread tools, edit title, add prefix) that will get more eyes on your post. :)

I am only about six months into this, so I am not nearly as experienced as lots of the members here who should be dropping by to help. For now, have you chosen a procedure to follow (either TR or SLGS)? I do not see it in your signature. That is my initial thoughts because this will determine when you make a reduction. You have only waited three days sometimes to increase, so I am thinking you are following TR. If this is the case, you reduce by .25 if they get under 50. BUT --- you also are only supposed to be increasing by .25, it looks like you increased from 2 to 2.5, and sometimes your doses aren't the same in one day. I have learned from others here that consistency is key. Once you do an increase, you keep them on that dose for both AM and PM, and wait three days / six cycles. At that time you can look back and determine what your next step will be. But for now, I think people will probably suggest getting a dose that you are comfortable with and stick with it for three days, then go from there. I am thinking 2.25, but I am curious what others will suggest.

In your last post here http://www.felinediabetes.com/FDMB/threads/glucose-numbers-not-making-sense.198346/ you mentioned following SLGS. Could you put SLGS in your signature if you are still following that dosing method? If you are now following TR
, please put that in your signature. Specifying your dosing method will help you get dosing suggestions.

If you are following SLGS, you would reduce the dose back to 2.0 units because Princess went under 90 yesterday. However, you have been increasing too fast for SLGS, which says to held each dose for seven days. If you were following TR, you wouldn't reduce until seeing an under 50 on the human meter.

Regarding bounces, cats bounce until they don't. We don't have much control over it, other than trying to keep them at doses where they see lots of normal numbers which will let their bodies get comfortable in normal numbers.

Welcome!:D You're in the right place for good advice and support. I have to agree with @Amanda & Shmee - please keep your doses consistent. Lantus is a depot drug - and changing the dose for one cycle isn't the way to help The Princess optimally...keeping your doses the same for several cycles - to be determined - can help the depot build up and you may start to see better numbers. :woot:

It's still early in the game for you guys since it's only been a couple of weeks. Excellent job on the testing and consistently timing the injections. :woot: If you have a chance look at some other posters here and their spreadsheets to see curves and dosing cycles. I found that to be very beneficial...we've got a ways to go ourselves :):bighug:

I thought when asked last time it was more SLGS but realized later it was more TR even though I started low at .5u because I had some strange results on Vetsulin early, if over 300 nadir it says increase .5 after (6) cycles which has been what I have been doing all along til I was under 300 and I changed to .25u dosage increment. I went to 2u yesterday because I was concerned that nadia was so close to PMPS, I guess I should have kept it at 2.25u since it was over 50 - My apologies!!!!

In regards to my questions,
  1. I realize the immediate high numbers of double digits was bouncing but is the current high numbers bouncing? or from the dosage reduction last night? or who knows?
  2. And in the future if I get low again (now I know to hold to 50 before reducing) do I continue to keep the same dosage the same all through the bounces?
Waiting for under 50 before changing dosage, especially with nadir so close to shot time felt like I was tempting fate with Hypoglycemia.

Bounces can last up to 6 cycles....I'd say she's still bouncing from yesterday's greens

And in the future if I get low again (now I know to hold to 50 before reducing) do I continue to keep the same dosage the same all through the bounces?

Usually the answer is going to be "yes....hold the same dose unless she drops below 50"
 
Going to do a full curve today, but thinking I should up my dose to 2.25 again (actually 2.125 looks best, but not sure how to do that) - Looking at my spreadsheet does this look like the correct move???? - I don't want her to go into Hypoglycemia but 2u does not look like its going to work.

You may want to post a new entry for today: Put your title like: 8/4 Princess AMPS xxx Time for an increase?
Select the '?' from the drop down list next to the title on the left.
Then ask your question about increasing the dosage.

I've been advised that it's always best to decrease/increase in .25 increments. You won't know the impact until you're testing, testing, testing...and can watch the drop and subsequent rise in the numbers. I just increased Luci this morning by .25. I used cailpers to find the excact dosage increments. New diabetes (and I'm still new after 7 months) should increase/decrease only by .25 increments...
