Hoping we're not in for another long haul with the low numbers like on July 19. Usually he comes up fairly easily and eats OK until he's on the rise again for real--let's do it that way today, all right, Felix?
 
If I may offer a suggestion... It looks like when you're shooting low, particularly during the AM cycle,Felix is dropping into the 40s by +2. You might want to consider testing at +1. Gabby was much the same, she would drop fast and early in the cycle. I would routinely get a +1. One other thought is to spread out Felix's food (e.g., feed him at AMPS, +1, +2 and maybe +3). This may help to keep his numbers bumped up.

Given Gabby's proclivity to drop like a rock, I would usually test every 30 min. Once Felix has earned a dose reduction, you can also be a bit heavy handed with the carbs. The goal is to get the numbers above 60 or so, that way you have a greater margin of safety should numbers start to drop again.

I had drawn up his new reduced dose of 2.25 units thinking he'd be starting his bounce, but he's still pretty low and I went with a BCS of 1 unit.

Thank you for the suggestions, Sienne. Regarding testing at +1, my first thought was "but he's always higher then," but then I looked over his spreadsheet and I can't say I have enough data to know that for sure!:oops: I did get a +1.5 one evening last week and thought maybe I should do that more often ... but I haven't yet. Felix typically does get most of his food across the PS to +3 time frame (plus more beyond that on days like today of course). I guess he technically didn't earn his reduction until he went below 40, although obviously I'd been giving him extra carbs for a while by then, so I already planned to give him a reduction tonight before I saw the 39.

I love that he is a very compassionate boy and earns the last couple during the AM cycle.
I'm always telling him how compassionate he is!

It is three separate days. Below are the guidelines for earning reductions:

Long term diabetics - more than one year since diagnosis
  • reduction *earned* after a single drop below 40 mg/dL for those following TR, or
  • reduction *earned* after 3 drops between 40 - 50 mg/dL on three separate days for kitties who have shown they do not hold reductions well and following TR, or
  • reduction *earned* when the cat regularly has its lowest BGs in the normal range of a healthy cat (50 - 80 mg/dL) and stays under 100 mg/dl overall for at least one week and following TR.
  • reduction *earned* after a single drop below 90 mg/dL for those following SLGS.

So far it looks like Felix is holding his reductions well. He is technically a long term diabetic but he has held each and every one of the reductions you've given him well . I would give the reduction if it were me. You probably could have shot the new dose of 2.25 last night's cycle , but if you needed some sleep, I understand why you did a BCS dose. I would shoot the new dose 2.25 this morning if you haven't all ready.
 
I just dropped a note to @Bobbie And Bubba (and thanks for the additional info!).

The '3 drops into the 40 -50 range' came from the Tilly diabetes page -- for those who are unfamiliar, this is the German Lantus group and where the TR Protocol originated. The dose reduction instructions state, "When the cat regularly has its lowest BGs in the normal range of a healthy cat and stays under 100 mg/dl overall for at least one week, attempt to reduce the dose. Alternatively, if the nadir glucose concentration is 40 - <50 mg/dl at least three times on separate days, try lowering the dose. If the cat drops below 40 mg/dl once, reduce the dose immediately!"
 
I started waiting for three drops under 50 or one under 40 last spring at Wendy's suggestion, but I've taken reductions faster than that lately because he's still riding low and has really seemed to want less insulin right now. That PMPS=108 made me nervous after the day we had, but when I saw how things played out last night, I wished that I hadn't done the BCS!
