Leo update -Triaditis, possible metabolic acidosis?

Hi everyone

On 3/5 I first posted about my diabetic/CKD cat Leo and gave details here:

Leo has been doing well overall but I would like some opinions. I have his latest blood work, and urinalysis in his spreadsheet.

First, a quick update...

On 3/21 Leo had an ultrasound and was diagnosed with Triaditis. As expected, it also showed kidney calcification. A urine culture was negative. He was given Metronidazole, Mirtazapine (for inflammation, not for appetite) and liquid Hepato. He also continued what he had been taking: Amoxicillin, liquid Potassium, Phos-bind, and 150 ml Lactated Ringers once/day.

A follow up ultrasound on 4/24 showed very good improvement. The vets were happy with his response to the meds. At this time, I thought he was losing weight. He seemed bonier. However, the vet's scales showed he actually gained a bit. Very weird.

A week later on 5/2, the same vet scales showed he had went from 8.4 to 7.8 lbs. He had blood work and a urinalysis (see spreadsheet). He stopped Amoxicillin and started Azithromycin. Lactated Ringers was increased to 200 ml once/day.

The next day (5/3) he seemed to have trouble peeing. He went from litter box to litter box, at first not peeing, then peeing enough to make dime size wet spots and finally going a decent amount. I took him to vet immediately. The vet thought Leo only felt as if his bladder was full and it could be a side affect of starting Azithromycin. He had a urinalysis (see spreadsheet).

This same thing happened again on 5/19. Over about 2 hours he was in and out of the litter box but peeing only a little. He eventually had a nap and then seemed fine. I assume this has something to do with the Azithromycin as the vet suspected?

Leo is scheduled for blood work and a urine culture on 5/30. I'd like your overall thoughts on what has gone on, the meds he's been given, his blood work, etc.

Also, I am very concerned about Metabolic Acidosis. I don't remember if it was ever mentioned/considered by my vet. I have read about it on Tanya's CKD site. I don't see a test for blood ph in Leo's blood work. His TCO2 isn't low. However the Anion Gap has been consistently high. Also, his urine PH is low (although I've read it's not a reliable indicator). Wondering what you all see in his labs and think about Metabolic Acidosis.

Sorry I don't know anything about Metabolic Acidosis.

About the peeing:

- did the ultrasound look at the bladder? any hint of possible stones?
- when you took him in on 5/3 did the vet palpate his bladder to check if it was full or not?

Metabolic acidosis is correct and it’s very different from DKA. MA is common in cats with CKD.

Trevor....His anion gap could be high because his phosphorus is so high. How much binder are you using because his P went up....a lot. That P really, really needs to come down because his P x Calcium is way too high. I’m not surprised you are seeing calcification. I still don’t get how his creatinine, BUN, potassium, and HCT are not that bad but his P, Ca are really way too high. If you can get the P down with much lower P foods, that would be good but I also wonder if you might need more binder.

MA will cause muscle wasting and the weight will usually drop. It’s hard to get an accurate picture of MA without doing the additional lab testing which is $$$ and the kitty has to be anesthetized so you might want to talk to your vet about treating for MA by feeding a kidney diet. You are already giving LRS which is another treatment and he’s on potassium.

On the urine issues, I wonder if he’s got crystals due to the potassium? That can happen. I don’t see a value for it on the most current urine labs.

Did they see any sign of kidney infection on the ultrasound because that can cause the creatinine to jump.
