I was given bupe for Teasel's sister after her dental this past Thursday: 0.2 mg/mL concentration, volume of 0.25 mL to be given every 8 hours. She's about 11 pounds. They gave me a marked syringe to draw the bupe into. I chose to give her half doses and she only received three doses in total.

Silver was fine from Wed to Sat. It was just from last night he didn't want to eat. He's eating FF in gravy right now. Breaking all the rules to make sure he eats.

If it's an infection at the extraction site that can take a few days to develop. If there's infection his mouth will likely be sore and he might not feel like eating. An infection is also something that can increase blood ketones. All speculation at this point. You just have to feed whatever he'll eat so you can give his full insulin dose.

That makes a lot of sense. I'm going to see if I can get antibiotics anyways even if he can't see any infection. I read in the manual that the ketone meter will read HI if it's over 8. Vet clearly didn't know when she said HI meant around 4.

Teasel's sister wasn't prescribed antibiotics after her dental because there was no evidence of infection in her mouth at the time of the dental. That was probably the case with Silver too. However, they eat once they're home and the extraction site, although sutured up, could develop an infection like any wound in a moist, bacteria-laden environment. I suspect it's also easier for that to happen in a diabetic whose blood supply has more glucose in it than a non-diabetic. Bacteria love glucose. High BG also causes poorer wound healing in diabetics.

He won't eat anything else. I tried. He's had a sardine; 1/4 can of Recovery and 1.5 cans of FF one of those in gravy. I think he's had enough that I'm not too worried if he won't take anything else. He had both doses of insulin today. I need him to eat in the morning before I go to work.
 
He's not doing okay. Would not take any supper. Gave him more fluids and more pain meds. Praying he is safe until I can get him to vet. I have texted work to say I will be late in and will take him in at 8am tomorrow morning. (Scared I am risking my job here) but I can't wait all day to take him. Am a little scared he might not be okay overnight but the logistics of taking him at this time of night to the emergency vet when they close first thing in the morning and I'd have to transfer him from one vet to the other are just too complex. Please, I need all the vines I can get right now for his safety. Not going to sleep well tonight.
 
Will he eat the sardines? Ice cream? Bread? Milk? Cheese? Anything? Try to get him to eat anything that is non toxic to cats. If the mirtz lasts 9 hours could you give him more (I have never used it and I don't know about dosing frequency)? Hopefully if he's just having pain in his mouth the bupe will kick in and he will eat soon. If anything changes, as much as it would be a nightmare to take him to the ER, that may have to be the way to go. I hope you can get him eating. I understand, I would be hesitant to give it again too, I don't know enough about it.

Sending all the vines he is okay until your vet opens in the morning. Our Toki had a severe allergic reaction in the middle of the night once and we didn't have an emergency vet option, it was a very long night until the vet opened and I was afraid she might not make it, so I know exactly what that feels like. Please leave him some overnight food options just in case he's up for it. :bighug::bighug::bighug::bighug::bighug::bighug:
 
I wouldn't give mirtazapine more than once a day. Has he been given any nausea meds, ie. Cerenia or ondansetron? I would try that before more mirtz.

Hoping for a restful night and that you can get Silver into the vet tomorrow morning. :bighug:
 
If you can try to assist feed him that would be a great option too. If you don't have a large oral syringe, try to hand feed him or make his pate food watered down and syringe feed him with a small oral syringe like came with the bupe (doesn't hold as much food and would take longer, but better than nothing). More vines for Silver :bighug::bighug::bighug::bighug::bighug:
 
I hope he will try to eat a little something for you , even if just soupy gruel.
Try and get some rest somehow & I hope Silver has a sound sleep too , ready for the morning visit.

I didn't give any more mirtz and wasn't given nausea meds. I've just emailed work to call in sick. I have to be with Silver today. He wouldn't take any more food last night @Stacy & Asia and I did try pushing sardines into his mouth. Not happening. Just woke up and so glad he's still with us. I thought he might not be. He's vomited up the last food he had. I think that's what woke me. Four more hours and vet opens. Once again DCIN have come to my rescue and will help. The dental and meds just cleaned me out. I hope the vet can help him. He's such an unhappy cat right now.

I’m so sorry, I know what a very long night it’s been. It’s almost time, hang in there. I would ask your vet to have some Cerenia (anti nausea medication) to have at hand at home so that if this kind of thing happens again, you can help him, especially if it’s past clinic hours and they are closed. I hope they can get him feeling better soon and will give him antibiotics this time. Sending vines that your job is understanding and that Silver feels better very, very soon. I think this is far beyond nausea tho. I have a feeling this is full blown DKA. Possibly due to infection after the dental. I have to trust the vet and do as they say.

It could have started out (the inappetance) as just nausea. I hope it’s not DKA. You do need to trust your vet, but it’s okay to question things they do too, Silver is your boy, if something doesn’t make sense or seem right, they should understand your concern and want you to be on the same page and answer your questions enthusiastically. :bighug::bighug::bighug::bighug::bighug:

Well they have to cal DCIN before doing any treatment so between them and the vet they won't go wrong. I am not a vet and I do not know what is wrong so I must and will let them do what is best for him. The forum is great but I am here alone with the vet so that's who is here and I have to let them do what they need to do for him without my interference.

Sounds like you're there at the clinic now. Yes, the DCIN people are knowledgeable - and thank heavens they're there to help! I hope it gets sorted soon and that it's not DKA. Keep us posted.
