am I doing something wrong with the dosing?

Lantus dosing isn't based on the Pre-shot numbers and you need to hold each dose for at least 6 cycles before making any changes (unless they drop below 50)

Since you skipped the PM shot on 2/2, that "cycle clock" should have started over on 2/3's AM cycle.

You need to drop back down to .1 (which you earned anyway by dropping below 50) and hold that dose for at least 8-10 cycles

INCREASING THE DOSE:
  • Hold the dose for 3 - 5 days (6 - 10 consecutive cycles) if nadirs are less than 200 before increasing the dose by 0.25 unit.
    • if your cat is new to numbers under 200, it is recommended to hold the dose for at least 8-10 cycles before increasing.
    • when your cat starts to see nadirs under 100, hold the dose for at least 10 cycles before increasing.
  • After 3 days (6 consecutive cycles)... if nadirs are greater than 200, but less than 300 increase the dose by 0.25 unit.
  • After 3 days (6 consecutive cycles)... if nadirs are greater than 300 increase the dose by 0.5 unit.

So the dose is based on the Nadir of the previous cycle?
 
Yes, you look at the nadir of the previous cycle. Unless he drops below 50, you hold the dose for 3 days (6 cycles) before making any changes.
