Paws is so energetic this morning, and I just want to be in bed!

I decided that I needed to give Paws 0.5U this morning (or as close to it as I can), just because I have so little data, his numbers fell so quickly the last two nights, and I don't have nearly the flexibility to monitor him today as I'd like. This early in the process I'd rather go too slow than have a hypo cat. If he had flat curves it'd be a different story but he's all over the place and I'm still learning. So we'll see.
 
Perfectly reasonable. I'm in the zombie club with you today, hope you can catch up on sleep a little bit. I would highly recommend buying some digital calipers such as these:

If you measure doses that way, it's much easier to be precise and consistent. I've had a 2.25 unit dose measure as 1.5 unit on the syringe and it can go the other way as well. Especially is Paws is playing with such tiny doses (under 1 unit), it will give you some peace of mind that you are giving a consistent dose. Not sure if you've encountered yet, but syringes can have off printed markings on them quite frequently.

Thanks everyone! I was able to get away and made a run out to get some syringes with half unit markings, and replenish my supply of test strips. Paws is now occupying his usual position of on top of my arms while I'm trying to work.

Thanks for the suggestion on the digital calipers, I'll probably pick up one b/c they're on a $9.99 special right now.

Can't believe how well the Lantus is working for him. My vet is really surprised too. Makes me wonder what's actually going on inside of him. His numbers are looking "safer" this morning - or at least way less volatile.
 
Paws has been stable blues all day so far. Assuming PMPS is still blue or low yellow, think I'll just stick with 0.5U for now?

Had to discuss blood tests with my vet; she recommended shooting 1U if above 200 and zero if below 200. Doesnt seem like sound advice (at least for someone not involved on FDMB) since last night he went 247 to 42 on 1U. But also not sure how to manage the vet in light of the advice and experience here.

Good news is his SDMA blood test result was 5ug/dl (normal!) so CKD is no longer a concern!!
 
I would agree, stick with the .5 unit for now, at least until you decide TR or SLGS, don't want to get too close to those lower greens until you know decidedly how to deal with them (dosing decision wise). But let others ring in, I'm pretty new myself, so that's just my two cents.

I don't think a lot of vets are up on the latest FD stuff. Some vets also maybe hear from colleagues or conferences that Lantus is the best insulin for cats and they prescribe it to be used like a short action insulin, unwittingly. Dosing a depot insulin like that is not a good idea. Vets also have to consider the time/cost factor involved in managing diabetes and I think they try to err on the side of making life easier for the humans, and less ideal control for the cats, to keep it simple and keep the cats in their current home as opposed to being surrendered or euthanized.

If you like your vet and you're generally happy with their care outside of the FD stuff, I would see if they are open to you using the TR protocol, or SLGS and would support you in trying that for control. For example, if you used TR, you could bring the published study with you so you have the science in hand, and let them know you have this board and are able to closely monitor your cat. Let them follow your SS and give weekly email reports/updates or something like that? After all, you both have the same goal (to keep your cat happy and healthy) and an awesome vet should realize there is more than one path to achieve that. Good luck!

Great news on the SDMA results!

I too would stick with the 0.5 units for now. It is so much easier to start a little lower than the right dose and work your way up. Seen too many baptisms by fire here with people starting on too high a dose.

Ditto the comments on dosing. Here we find the depot insulins crave consistent dosing and it makes it easier to make dosing decisions if the depot is stable. I gave my vet a copy of the dosing methodology paper and copies of the spreadsheet for the first little while, once I went "off book" from her dosing. As for shooting lower numbers, look at some of the other posts here to see what happens when people shoot lower numbers. It can be unnerving at first, so we suggest not feeding and posting for help the first time you see an under 150 preshot

Even though he dropped below 50 last night, you might want to consider holding on to the 0.5u dose, the numbers last night are likely influenced by the depot of the 1u. (it can affect up to 6 cycles later).
Going down the dosing ladder too quickly can be counter productive and result in you having to go right back up (usually beyond the point that you first started to see progress)

I see he is blue this morning, so numbers on the rise, as long as you can monitor (I see you joined our FD testaholics anonymous:)), I would suggest you hold on to the 0.5u
