Inflammation- Ches back in hospital

After a long night and run to ER vet at 3am on Sat morning, Ches was readmitted to hospital. He was definitely in pain :( Once Bupe was administered he was more comfy and wolfed down food.

Inflammation has been playing hell with his bg numbers. I saw him tonight and vet reported that his numbers dropped to 20 and then shot back up to 30 :(

Any ideas re what to do about the inflammation would be appreciated. Trouble is we can't pinpoint WHERE it is. App his amylase doesn't suggest Pancreatitis. That's not to say he doesn't have symptoms. Total intolerance to fat. Blood actually goes milky.
When he is eating he is packing away at least 250g. But he doesn't seem to be able to do much with it. He is now 3kg so has put on 3g in a month. His BM's are formed not projectile, so an improvement I guess.

Any suggestions? My heart is sore :(
 
Could the vet put him on a long term low dose of metronidazole? This is an antibiotic but also has an anti inflammatory effect in the bowel. They prescribe it for my dog when she has flare ups. It might help and is very cheap. 10 day course for Rozie was like £7 or something.
 
No idea @TempestsMum, worth an ask. He's already on Convenia - happened before I could say anything :(

I was wondering if there was ANYTHING other than steroids that might help with inflammation.

I'm speaking to the vet tomoz. See what she says. To my mind, it's not worth him coming home until this is under control.

Convenia is an antibiotic isn't it? I do know that it works long term like 14 days but can have bad side effects on some kitties. I think it can stay in their system for like a month or something.
 
Amylase is not a good indicator of pancreatitis in a kitty. Ask if they have the snap fPL test for pancreatitis. It will only give a "yes" or "no" answer but at least you will know if pancreatitis is at play here.

Thanks @Tuxedo Mom That was a "gut" (no pun intended) feeling I had, given the signs and presentations.

I am now going to push for the IDEXX test - not Snap. Might as well go the whole hog. Means sending it to the US, but hey, least I'll get an answer. I don't see why not, given I'm paying the bills :-/ xx

The IDEXX test is much better since it will give you an actual reading. But for a quick test most vets will have the snap fPL test which will at least give a yes or no answer right away.
 
Good thinking :) I just feel after the raft of bloods they've done, it's the one they've not done :(

Any ideas on IBD tests? I know what it is for humans lol.

Or does Snap fPL cover all inflammation?

IBD can be checked with an X-ray or ultra sound. If this shows possible then the best way to know for sure is a biopsy.

The snap fPL only indicates whether pancreatitis is present.

ETA

IF IBD is diagnosed usually a steroid is used. Budesonide is a good steroid for IBD since it has less systemic effect on the glucose levels.

I hear you. There is always a possibility that Chester's body does not like certain types of food. Sometimes trying a novel protein can help to rule out whether a certain protein source such as beef or chicken or fish etc is causing inflammation in the bowels and digestive tract.

I have Ches on Bison for that reason :) I steam it so hopefully lower any more fat in it. It's low carb, low fat, nutrient dense. Since starting on it his BM has improved to bring acceptably formed.

It's not an ideal diet, but it's a start. He def doesn't tolerate raw or anything with a moderate fat content.

Sounds like a good choice. Good source of protein and low fat and something he probably didn't have before you started trying novel proteins.
 
BM's are very dark to black tho :( So a real indication of inflammation / bleeding :(

SEB I understand may be an option, tho human Diabetes sure says it may inhibit Glucose absorption? But if others have used it (I did try it with him) I Obs don't mind trying.

I used SEB with both my FD kitties. My Tuxie (GA) had chronic pancreatitis so if he didn't eat everything in sight I would get very concerned. SEB helped a few times with him. When he had a pancreatic attack cerenia, buprenorphine and Sub Q fluids helped a lot.

How long did the flare ups generally last?

In 2 1/2 years I was very lucky he had 4 flare-ups that required medications. Since I caught it quickly each time...when not eating everything in sight...I went to vet immediately...it usually only took about 3 days until he was back to normal. His fPLI was off the charts...but he also had Cushing's which made everything more difficult.

Are you adding supplements to the bison? A pure meat diet doesn't have all the nutrients kitties need unless you're also feeding the organ meats like liver, heart, kidneys

Thanks Chris. I'm well versed in raw diets. :) :) It would be my ideal to get him onto a raw diet, if he'll tolerate it, one day :) My other cats are raw fed.

I have asked about safe supplements, but not really had anything useful back :( To be fair, I haven't asked in here. :)

The reason I haven't gone all out, yet, with Ches is because I only started on the bison (steamed, not raw) last week. My rationale was, one thing at a time.. Just trying to get a firm BM in itself has been a major victory.

Do you know of any safe supplements?
 
The one a lot of us are using is EZComplete, but I don't know if you can get it in the UK

The most important thing to make sure to add is Taurine.....without that amino acid, they can go blind and it's also important for their heart.

Thanks @Chris & China . Taurine, B vits and Potassium I had covered in the Kaminox supplement he was having :) It was the rest I needed to cover. :)
I think we can get EZComplete in uk. I'll do a search for it, thanks :) x
 
Can he take probiotics? That might help him some. It helped my IBD kitty. If you are looking for other foods to try, venison is also pretty low-fat

When I fed venison, all my kitties had very dark brown/blackish BMs, scared me until I realized it was because of the venison. I think it has to do with the type of meat, so maybe some of Ches's is the same, from the bison?
 
He's on Bioglan probiotics. 20 billion live cultures.
Mix of lactobacillus acidophilus, lacto rhamnosus, bifidobacterium longum and S Boulardii..
