? 8/22 Abby Day 4 of Increased Dose & Still Wonky

The first day we increased Abby's dose (8/19) her numbers started to come down to the 230-290 range. But since then they have gone up and are remaining stubbornly high, even higher than on the lower dose! :banghead: Should I hold her a few more days at this level or is it time for an increase? I guess I was anticipating that her numbers would come down a bit for each dose increase, maybe bouncing up a little for a day or two then coming down. I'm confused why they would go higher.

Hi there, the PM cycle of 8/19 there are no test after the PS number of 292 so maybe she went lower into the blues causing her to bounce the next day. The bounce could take 6 cycles to clear. I think what I would do is hold off an increase until till tomorrow morning and try to get as many test in today and tonight as you can to see she could be going lower at night and then bouncing up.

Why the 0.5u increase on 8/19?

Did you decide to follow the TR protocol? If so, please note the below information regarding increasing :
  • After 3 days (6 consecutive cycles)... if nadirs are greater than 200, but less than 300 increase the dose by 0.25 unit.
  • After 3 days (6 consecutive cycles)... if nadirs are greater than 300 increase the dose by 0.5 unit.

I agree with Bobbie regarding holding this dose until you find out where it is taking her between PM+4 and AMPS. By not having any data during those hours, you are missing 1/3 of the picture. Many kitties go lower on the overnight.

To answer your questions, yes I am trying to follow the TR protocol as closely as possible given my work schedule. The 0.5 increase (versus 0.25) was for two reasons: 1) my vet actually wanted me to increase by 1 unit so this was my compromise with her, and 2) I'm really having trouble measuring 0.25 on my syringes with the U-100 insulin. Here are the ones I buy: https://www.adwdiabetes.com/product/1289/monoject-ultra-comfort-insulin-syringes ...I thought with the half-unit markings and the 3/10 cc size that I'd be all set. That size worked great for me with the U-40 syringes when we were on Vetsulin. But the markings on these U-100 ones are so darn close together even with my sharp eyesight I can't manage to hit the 0.25. Is there another kind I should be buying? (Is 3/10 cc the smallest size available?)

Could you put "TR" in your signature block, so we know that's the dosing method you are following? Many of us use magnifiers or "cheaters" to help with dosing. That and a good strong overhead light with the syringe over something white helped me. The 3/10cc are what we all use. Another trick some of us use is to dose with calipers.

I see a note on the spreadsheet about possible sandwich theft - that will definitely raise the numbers. Bread is high carb. :p
