? 8/17 Post DKA Millie AMPS 329; Trying to find best dose.

Trying to figure out what is going on with Millie and find the best dose
for her. She was running very low on 1.25units of Lantus (bg 35!). She is now on 1u but she seems to be 'flat'? Not sure if she's bouncing a bit or what. Trying to give her fat 1units but not sure if it's working. She is very sensitive to food and currently being kept on a mix of LC & MC food due to the DKA. Not sure if some readings are food influenced.

Feedback appreciated.

My take on what's going on is that it is quite possible that Millie goes lower at night and then bounces up by AMPS. Example: The PMPS on Aug 15 was 281 but the +2 was as low as 228 - quite a drop! That alone would provoke bouncing in my cat and many others, too. That fast drop by +2 usually indicative of an active cycle - I would make sure to get in the +4 test at least. It is very possible she went low - many cats naturally go lower at night, and than bounced up to the Aug 16 AMPS=326. It is my $0.02.

I hope someone with experience will comment on the dose. I'd try to catch her low at night and then reevaluate the dose. But because Millie is post DKS - an increase may be the best way to proceed.

I agree, I figured she probably went lower at that cycle. She gets her dose at 9pm, medical issues make it tough to test at night but I'll try.

I too shoot at 9 PM and believe me I know well how difficult it is to either stay up or to set an alarm for 1AM to test at +4. :blackeye::blackeye:
But you do not have to do the +4 every night.:) Take the PMPS, then +2 every night and if the +2 is the same or lower then make sure to test at +4.
On the nights when her +2 is higher than the PMPS most likely (but not guaranteed) the cycle will stay flat or be a bouncing one.

I'll try to do that. Unfortunately if I wake up, I stay up (sleep disorder). I did decid I will do more night time testing one night this weekend & then try to catch up on sleep the next day. I know we need to figure out what is going on with her so I know what to do with her dose. I bought a caliper and I'm waiting for it to arrive. Maybe that will help with fine dosing.

I should have added: doing the PMPS +4 on a bounce cycle won't give you any useful information... Do not risk ruining your sleep if the +2 much higher than the pre-shot. Try doing it when you get the +2 lower than the PMPS.

Have you ever tried Valerian Root for sleep disorder? It is mild and won't make you foggy in the morning even if you take it late at night. I liked it! Just make sure it out of Millie's reach - some cats go gaga over it!

Thanks for the heads up, re: bounce cycle (this is the type of info I need!). And yes, I have taken Valerian, it's been helpful. Didn't know about the kitties though, lol!
 
She may be going lower at night but based on what you have she needs to go back up to 1.25u especially with prior DKA . It's common to go up and down in doses. Hope you are testing for keytones regularly. At least one test at night, you can scatter them is essential otherwise you only have 1/2 the picture.

Yes, I'm going to try to do a little more testing at night to get a better picture.
 
It's hard especially with sleep issues. I personally would bring her dose back up to 1.25 though.
I am going to get more night time readings over the next few nights before increasing her dose. I get nervous because the last time she got DKA I had increased her dose without having the whole picture. She started bouncing and didn't stop (BGs in the 500 & 600s), then she started developing ketones and ended up in the hospital. I also realize she needs to be getting enough insulin to keep her bg in control. A fine balance.
