Good morning everyone!!

Han Solo is acting again, even though he started quite high yesterday we had two very nice cycles, but this morning he was 135 so Fernanda skipped the shoot.

My question and I hope someone has some suggestions is what do we do for the next dose try and hold the 2.75 she gave yesterday or go for a decrease? I wish we could find a dose that allows to shoot more constantly
 
Is Fernanda using a human meter or a pet meter? Also how much is she able to test when shooting lower numbers? I know Hans Solo is hard to test, but skipping those lower preshots only puts him back into higher numbers at the next preshot and messes with the depot....I believe she is using Lantus???

Yes she's using Lantus, and a human meter

The real issue is exactly that we are really not sure if she's going to be able to test properly is more a matter of luck last night she caught one test at +4 and she tried for over 30 minutes at +7 to try and get a test with no luck so he may have gone lower that that 132 and odds are he was on his way up this morning but we are not sure, also she tried to stall and retest but is hard to get two tests within a small amount of time ( he moves, scratches bites, etc).

I know she's tried the "taco"/"burrito" but he sruggles too much, It looks like he may be starting to accept the testing a little ( with some help of very nice rewards) but they are not there yet.

One thought, which is against all L/L protocols, is to give at least a token dose when the preshots are lower...that way there would be some insulin at work rather than skipping the shot altogether. The only other thought is that if she can't get Hans Solo on board with consistent testing, is there anyway she could look at changing to a different insulin such as Prozinc, which could be given at different doses if need be. I am sure most experienced Lantus users will disagree with both thoughts, but trying to get Hans Solo in consistently good numbers with possibly erratic testing is a tight call.

Unfortunately Prozinc is not available neither here not in Argentina where Fernanda lives so not an option.

I've been thinking we have been going down from her initial dose which was way too large as we've seen what if we went much lower like 2 units (also not really within protocol) and started going up.? Again just a thought

His numbers have been much better on the lower doses, even with the skipped shots. I would tend to agree with a lower dose like 2 or 2.25 ??? units (again not within normal protocols)...only because of the problem with testing. It would be better IMO to have consistent dosing AM and PM and not have Fernanda skipping because of lower preshot numbers that she is not comfortable with. With any luck Hans Solo will become more complacent about testing and then the usual protocol can be followed. It would be better to have both AM and PM doses given than have a higher dose and have Fernanda afraid to give shots every other cycle.

Just to clarify for anyone reading this. This is NOT the normal protocol for Lantus, but with the big problem Fernanda has with being able to test and her concerns about shooting lower numbers this is a thought on an alternative approach. Also all information is being relayed to Fernanda through Veronica since Fernanda does not speak English and the translators make it hard to give accurate information and ex[;anations.

Add Hans Solo info into your signature, and as link. Would it help those that are reading about him, so they don't have to go through as many condos to get info, and you don't have to repeat info/links as offer.

I would not add Hans Solo to your signature Veronica, as he is not your cat. I would only want people commenting on his dose that know his history. What we are suggesting for Hans Solo is unique to his situation only.

I agree that he's been getting nice numbers at this dose, but with testing difficulties that can be risky. It's too bad she had to skip cause he will likely zoom up for tonight. She may just want to go down to 2.0 units until testing becomes easier. And hopefully that over time testing will become easier and she can increase again. If she needs to.
 
How is Hans Solo doing? numbers seem to have stabilized, maybe a missed reduction yesterday.

He's been doing quite ok, seems we may be able to finish his first week with a constant dosing actually we have made it through 10 cycles with 1.5 u, he did went a little bit low yesterday but it seems he had not eaten for a few hours when Fernanda made the test and since it was really close to 90 we decided not to change the dose ( taking into account he had not had food an all), given his history I was thinking that if nothing else happens it will be better to wait for 14 cycles to really give him time to get used to a dose and then evaluate her next steps

His SS does look much smoother than before.... hope he is also feeling and acting better.
