DOSING FOR BAXTER

I am not familiar with Baxter's food - Grain free Purina ProPlan canned. Is it LC or ..?
How is his appetite? If we 'll need him to eat will he do?
 
It might be the food talk.
He ate his preshot meal, right/ This BG might be influenced by that.
Are ok to take the +4?
Yes... I will do the +4. Why would it go up instead of down? He last ate around 6:30pm or 3-1/2 hrs ago. It was his low carb food (canned)

I'll try to check on you as I can - we have painters coming in the morning so DH and I are busy emptying the rooms. It is possible that Baxter is bouncing from that low this morning. It's a good idea, when you get a test that surprises you - high or low - to recheck right away to be sure the test was accurate. Looks like you just did that test 8-10 minutes ago, so you might recheck now to see if you get a similar number.

I suspect he will bounce through the night. By any chance, have you given his fur a sniff? Lantus has a very distinctive smell, and if you had given a fur shot, you'd smell it - it smells like Band-aids.

Interesting. No, don't smell that. I don't think I gave a fur shot as I always feel to see if area is wet. So his numbers are increasing for now due to bouncing from that low today??

Hate to tell you but it might go on for a while.
I am sure you know it but feed him as usual regardless of high numbers - some ppl are compelled to skip a meal if the numbers are too high.
 
I would say you could go to bed. Bounces can take up to 6 cycles to clear, but ECID and looking at Baxter's SS, he clear them faster than that. I've asked a veteran or two to chime in on your new dose for the morning. Hopefully you'll have some feedback before you shoot.
 
Ha... yes!! I was just going to say.... is it safe to go to sleep? I will probably wake up off and on and will test then.
I think you can stop testing until +10 or +11, and of course the +12 preshot.
Do you have Neosporin ointment with pain relief? Baxter might have some use for it tonight...

Wow.... up to 6 cycles to clear??!! So crazy. Cats are SOOOOO different than humans with diabetes. I know that's an understatement! Lol. We'll see what the rest of the night holds and I will ask for feedback before morning shot. Thx again!!
 
I think Sandy meant to tell you to post your questions/concerns earlier so the people from the East Coast can reply and you'll get some info before shooting AM. Maybe I am wrong.

If you are able a PM+11 test will be helpful.

That 43 AT reading indicates he is over dose.

How much? A couple things to consider-
Since on 04/28 the 3.5u dose took him from PMPS 594 to 79 by +6, I think 3.5u is too much.
While on the 4th cycle of 3u he saw blue and that dose may have shown some more action had the bounce been given more time to clear.

I peeked in on your SS at 2 AM EST when I got up to feed Bubba and saw that Baxter continued to rise . Now, to decide what is going to the best dose going forward. The bounce from the 79 during the PM cycle of 4/28 would have probably cleared the PM cycle of 5/1 but that was a fur shot and then you increased to 4 units for 2 cycles so, it's making discerning the best dose a little muddy. And as Sandy said above the 3 units was looking promising but didn't get enough time to clear the bounce.

ETA: Still get a AMBG test to see if he is continuing down or coming back up.

I will definitely get a AMPS and then test 2 hrs after......but I won't be able to test most of the day so I hope his numbers just trend up before I leave

I would not shoot any more than that. Unless someone with more experience than I have suggests you reduce more, I would go with the 3 units, as long as you can monitor. Keep in mind he's bouncing now, and we don't know if he will clear the bounce today, tomorrow or the next day - bounces can last as long as 3 days. Unless he goes below 50, you want to give him enough time on this dose to show what it can do. That means 3-5 days at least. From the sticky on TR:

  • Hold the dose for 3 - 5 days (6 - 10 consecutive cycles) if nadirs are less than 200 before increasing the dose by 0.25 unit.
    • if your cat is new to numbers under 200, it is recommended to hold the dose for at least 8-10 cycles before increasing.
    • when your cat starts to see nadirs under 100, hold the dose for at least 10 cycles before increasing.

Baxter was definitely under 100 today, so hold the dose at least 10 cycles (5 days) unless he goes low again.

My opinion: If you were going to be home to test and monitor all day, I'd say go with the 3 units, for now. Since you are not going to be home, and it looks like he's coming down from a hard bounce, I would go with the 2.5 depending upon his AMPS number or course. I'm sorry but I will not be here to help you at AMPS as I have to leave for work myself.

His AMPS was 364. He had eaten his low carb food. Should I give 2.5 units to be conservative?
I think I would go with Dyana and Sandy's suggestion on 2.5 as they have lots more experience than me and in lieu of the fact that you can't monitor today, I think that is what I would do. Still get a +2 after Test/feed/shoot (T/F/S )
