? Decreased needs - where to go from here?

Hello again, everyone. I have more data now than when I posted last week and I'd love thoughts about where to go from here.

This week and the following weeks will be busy - Jonathan is out of town and I won't be home all day to check Kahn. I want him to be safe and the numbers from last week concern me.

Does anyone have any suggestions?

I think you can stick to the .75u and leave out MC and or HC food. It's always better to be safe while your unable to monitor.I noticed on the SS you jump around on the dose amount. Levemier works best if you are consistent on the dose Following the SLGS you need to

Hold the dose for at least a week:
  • Unless your cat won’t eat or you suspect hypoglycemia
  • Unless your kitty falls below 90 mg/dL (5 mmol/L). If kitty falls below 90 mg/dL (5 mmol/L) decrease the dose by 0.25 unit immediately
The Fancy Feast Classics are lower in carbs. I see your feeding Fancy Feast Pate I thought that was higher carbs?

That is great that you were able to update the SS the data is so important to managing Kahn's FD!!

I've been tweaking because of the lower range BGs - going from 1U to .75U because of the 48, and from .75U to .5U because of the 118 that I was afraid would keep dropping.

That's where I'm confused - if his AMPS/PMPS are below 120-ish and he's having lower than I'd like to see numbers during other times, I'm not sure if I should stay the course with .75U or drop down a bit more?

Oh! I wasn't clear - I am feeding the "classic pate" - no gravy, "shreds," etc

Kahn hasn't been as thrilled with his wet food lately so I checked the newly updated list here and ordered a few other options to try, too. All are under 6g CHO. They should arrive today or tomorrow

Since you are so new to Lev, we recommend you not shoot anything under 150 unless you have supplies, are able to monitor closely AND have someone on the Board that can stay with you in case Kahn goes low. I know Kahn isn't newly diagnosed, but Lev is different from your previous insulin and therefore we kind of treat it like he's new. Remember that dosing on Lantus and Lev is based on how low the cat goes, not on the AMPS/PMPS. So while Kahn earned the reduction to .75 with that 48, he technically didn't earn the reduction to .5. We'll call that a BCS (Big Chicken Shot - one time reduced dose). If someone had been watching you, we would have told you to shoot the current dose and monitor closely. As it turned out, he did great, so no harm no foul. Going forward, though, you would hold the .75 until he goes below 90. If you get a PS you aren't comfortable shooting the whole dose into, stall and post for advice. We've been a bit thin of veterans here lately, but hopefully that will change soon, and you'll be able to get a response from someone that can help.

How is the weaning off of the dry food coming? Once you get him off of it completely, you can switch to TR, which allows reductions to be earned after 6-8 cycles. I get the feeling Kahn isn't interested in staying on insulin for very long, and once the dry is gone, we should see more reductions! :)

Got it on not shooting under 150 unless I'll be around to check/treat lows

Kahn has been on Levemir since the end of September, so for quite a while, if that makes a difference

We have three cats and they're down to about 2 cups of dry food a day between all of them. It's Orijen Six Fish which is 16% low glycemic carbs.

If we remove all dry food, it will have to be when I have a break between semesters and can be home all day to check BG
