I think I need to increase my post DKA Millie's Lantus. Her numbers have been consistently high. I'm also thinking we may need to change our protocol fro SLGS to TR. The SLGS seemed more comfortable when we first started but I don't think it's what Millie might need as a DKA kitty. I'm afraid I'm not getting enough insulin into her. She hasn't had any green numbers in a while.

Is an increase a good idea? I tried to stay up last to get more night readings.

What is the best way to change protocols? Anything I should be aware of?

TR might help her out because you can increase after 6 cycles if numbers permit and she has to get down to 50 on a human meter before a reduction is earned. I suggest you read over the TR guidelines to familiarize yourself. You're able to test enough for TR. I think you can do the switch right away but maybe the pros have a different take on it.

Millie has just been consistently high, but she is feeling good. Yes, I was just reading over the TR protocol. Trying to figure out how to print it so I can refer it a bit better.
 
If you don't get a response and want to start TR today, maybe increase to 1.75 u at this dose and indicate on your SS that it's the start of TR. You'd then test as usual and hold the dose for 6 cycles.
 
As far as switching from SLGS to TR, I think you just make a note on your SS and update your signature that you are now doing TR and just make the change. It looks like you test enough to do TR. You evaluate dose changes after 6 cycles on TR instead of a week and the number you earn a reduction by goes from 90 to below 50.

You definitely test enough for TR and when you can't you can deviate as needed. I do think more juice was needed. Except for high dose cats or cats that react to small amounts .25 increases and decreases are the way to go. Good luck I hope you see lower numbers.

Millie definitely reacts to to small increases, she'll probably bounce initially but she needed the increase.

So Millie's +3 on the new Lantus dose is 206 (AT), I'm afraid she's heading low or for a bounce. Have to step out for about an hours but will be back to monitor and test her again. She sleeps a lot during the day so not sure what to look for regarding hypo........
 
Not all cats show symptoms of a hypo if they get into low numbers. Testing is the best way to catch low numbers.

Here are the symptoms of a hypo in case you ever need them:

The symptoms to be concerned about include but are not limited to:
  • staggering, uncoordinated movements, 'drunken' walk, wobbling, balance problems
  • ataxia - usually lack of muscular coordination, but maybe changes in head and neck movements
  • disorientation (yowling, walking in circles, etc.)
  • twitching
  • stupor
  • convulsions or seizures
  • coma
