? 4/25 Utyun Please give me dose advice.

Hello!

Utyun is today AMPS 408, +4 55, + 5 120, + 6 180
It has gone down to 55 with +4.
Should I reduce the dose of insulin?
Is it common to suddenly fall down from 400?
Please give me advice.
 
Hi Kaori.

Are you trying to follow the Tight Regulation protocol or Start Low, Go Slow? With TR, you would reduce after a drop below 50. With SLGS, the reduction point is 90.

Yes. That 408 was probably a bounce from the mid to low 100s of last night. I would say you need to keep testing the way you are so you catch any other drops! If you are able to get a +2 test, it will usually tell you if the cycle will be active, and if you need to test more. If the BG is lower than the PS, keep testing. If it's higher, it is most likely a bounce and you probably only need one other test or so - around +4 or +5 if possible. If it's the same, it could go either way, so I always tested again at +3.

Don't be lulled into a false sense of security by a high pre-shot number. It looks like Utyun is a bouncer. :eek:

Hi Kaori, I was looking at your spread sheet and noticed that you are changing up the dose. The dose needs to be held for a required amount of time according to the method you are using either TR or SLGS. For instance, on 4/16 he gave you a test of 63 and 93 in the AM cycle. You gave .75 units that cycle and in the PMPS cycle you increased to 1 unit. I noticed a few other times you have changed the dose from AM to PM. Lantus likes consistency with the dosing and it needs to be changed in a systematic order for increases and decreases. Otherwise, the depot gets messed up.

Correct. You want to hold the dose according to the protocol. Which method are you following? TR or SLGS?
 
Tonight's PMPS was 492.
It is also expensive. Every night the night is higher than the morning.
Can I continue with 1 unit?

It looks like you test often enough to follow the TR protocol. With TR you can move the dose up faster which allows you to get Utyun into better numbers faster. But, you need to be comfortable with your choice.

That I understand. :) Don't worry, it's because he hit that nice lower green number today and is bouncing. It looks like he hits his lower numbers in the AM cycle and then bounces higher at night. We has a saying around here: Cat's bounce till they don't. Some cats when their bodies get more used to being in normal numbers stop bouncing. Other cats bounce all the way into remission. :cat:
 
I was at a loss which way to do.
If you can continue to test the way you have been, I would suggest TR. As I said above, you can get Utyun into better numbers faster and TR has a higher remission rate than SLGL but, with that said, we have people here that have used SLGS and have achieved remission.
