The sticky states if nadiars are above 150 shoot .25 can you monitor tonight? If you see good downward movement today lets re evulate his dose. When the depot builds it will release and Rudy is on cycle 6.
Hopefully others will have advice too.
  • If nadirs are more than 150 mg/dl (8.3 mmol/L), increase the dose by 0.25 unit

Right. But the general consensus from other senior members on here was that his starting dose on Lantus (2 units) was too low when he switched from ProZinc (4 units). There was no real bg movement with initial lower Lantus dosing and because of this his last 2 dose increases were both .5 vs. .25. I was thinking the increase would be .5 again. His numbers are still 300s and a .5 increase gets him closer to what his Lantus starting dose really should have been. That make sense?

I think that would be good to increase to .5 you can always lower. Especially if you can monitor. It would be better to get Rudy out if those higher numbers.
 
I agree, I would go for the 0.5 unit increase. As you get closer to the ideal starting dose for Lantus, you may have to switch back to the 0.25 unit increases. But I'm hoping you'll be seeing blue before then.
 
Thank you. I'll move him to 3.5 units tonight and see how he does. I'm hoping to finally see colors other than pink on his ss. :)
