Good morning everyone,

Ducia and I are proudly reporting our first night in the Blue! :D

There also were much less blood shedding done on my part as I learn to go by without excessive testing.:)

Ducia is doing great. It was her first night with only Blue number and what is truly new it was her first PM cycle with normal, not bouncy respond. It feels very good. And her energy level/ acting is really at its best when she is low - mid 100s.

But I think I might have under dosed her by leaving a drop of insulin in the syringe...:banghead:. It is the second time in last couple of weeks and I am really frustrated with myself this morning:mad:. Bad practice.

Another good news is that Ducia doesn't have pancreatitis. :D I received a call from a tech from the clinic who put it plainly "Ducia doesn't have it". I should have the test results later on and will update the SS/LAB and I would really appreciate experts feedback @Marje and Gracie :).

Thank you! It does feel good to see her in Blue and me not in the panic :):rolleyes:.
I am currently re-reading both FDMB protocols, trying to figure out which one is better for us. :bookworm::eek:. I will post later on to solicit opinions on modification that people do to protocols to accommodate cats/ beans needs and I would really appreciate to hear your opinion.:)

But first - trying to figure out the glucose toxicity, a new (to me) enemy that might struck if Ducia stays in Yellow for too long. I must say - Google Translate has A LOT of room for improvements :).

I would be proud if only it doesn't take poking her ears every other hour.:(

The actual result that I wanted to share reads:

TLI is 15.2 with ref range in ug/L 12 - 82;

fPLI is 1.9 0.1 - 3.5

Interpretation: serum fPLI concentration is within the ref interval; it is unlikely that this cat clinically significant pancreatitis, and then advise to investigate other diseases that could cause clinical signs observed.

I wish they put it plainly "No pancreatitis". Period.

But other than that we are very happy and wishing you all good surfing in the the green and blue lagoon!

Is it just the TLI and fPLI you wanted me to look at?

As you know, the fPLI tells if the kitty has pancreatitis and thenTLI is for diagnosis of exocrine pancreatic insufficiency so it's great news she doesn't have that....it's a nasty thing to have.

I am trying to figure out if Ducia's recent unsatisfactory response to Lantus is to be blamed on glucose toxicity...:bookworm::confused:

The more I read the more confused I become:oops: and there is no reliable translation for cats in my native language.

If you look at Ducia's LAB again, please, can you tell if there is reasonable suspicion for Amyloidosis?

Her ALT is elevated. One of the urine tests shown some protein problem, if I am not mistaken.
Does it give me reason to worry about hepatic lipidosis? I won't be able to afford any additional tests at this time.

If the glucose toxicity is a reason for poor response ( like this AMPS 332, +2 283, +4 189, fed lower than usual carb%) then I think I should get her some antioxidants in addition to Milk Thisle as you suggested, and I will have to consider another dose increase. She is a little bit below her optimal weight.

Amyloidosis in cats is rare and I doubt very seriously that that is what you are seeing. Her urine specific gravity is pretty high (normal) and if there is even a small amount of protein in the urine, the high USG makes it appear higher than it is.

I'm curious at the values for urine protein on her SS because it's usually reported as 2+ or 3+ instead of a number like 100. The test for protein in the urine is not always accurate and you can see some false positives....in fact, theynare common in cats. The other things that can cause protein in the urine are bladder or kidney infection but her other numbers in the urinanalysis don't look suspicious.

You can easily follow up on this, when you can afford to, by taking a urine sample to your vet. If you can get a collection tube and pipette from your vet, then when Ducia squats to pee, stick a large spoon under her and catch the urine, use the pipette to put it in the collection tube, store the urine in the frig until you can run it to the vet (the sooner the better). The vet can send it off to the lab to see if there is still protein in it, and if there is, ask for a urine protein:creatinine ratio but, truthfully, her USG being at such a normal number would indicate there is no kidney issue going on. I have to wonder if the presence of the ketones could have affected the protein test.

With hepatic lipidosis, one usually sees the alkaline phosphatase elevated and the ALT, AST, GGT might be variably high, low, normal and the bilirubin high. You should talk to your vet but I just wonder if her ALT is just slowly returning to normal because it has improved a great deal. BY the way, elevated liver enzymes are fairly normal when cats have had DKA, pancreatitis, etc.

I am not seeing glucose toxicity at this point. I think you just haven't hit her optimal dose yet. She is getting into blue numbers regularly and then bouncing as her liver gets used to the lower numbers. I think if you increase the dose per the TR protocol, you might her getting into better numbers sooner rather than later (we hope). Since she is seeing blue, you'll want to hold this dose at least ten consecutive cycles.

I think she's looking pretty good! You're getting there and asking the right questions.
