Frustrating start

Hi everyone. This is more a vent than anything else, but as always, I really appreciate any feedback.

I finally started TK on his insulin. Lantus from Marks (thanks for all the suggestions!). His pre-insulin numbers averaged around 250 and the vet recommended 1u, 2x a day. My instincts told me this was still a little too high, but I love my vet and went with it. Now, I've worked with animals for 10+ years and know better than to go against my instincts. But I've never worked with a diabetic cat before, so I doubted myself.

Anyway, first dose at 1u in am on Thursday. PM pre-shot read was 122, which made me nervous so I adjusted his dose to .75u and checked him later. At +1.5 he started acting slow, was at 70 and I gave him a few treats. At +2 he was screaming for food and at 30. I gave him some of his canned food and a little syrup and he was fine within 20 minutes (re-checked a few more times too). So not a big deal and really a mild hypo. I know a lot of you have been through a lot worse and I feel silly for even complaining. But you know... the feelings! :arghh:

I decided to lower his dose to .50u going forward. My vet is off Fri-Sun, so I haven't talked to her about this yet, but I didn't want to start during the week when I couldn't be home to monitor him. Now his numbers aren't budging much at all and I'm thinking I maybe should've just gone to 0.75, but would rather start low and adjust than the alternative. And I'm just kicking myself that I didn't stick with my instincts from the start and push my vet a little more on the dose! :banghead:

Venting is allowed! Is your kitty on low carb wet food only? I ask because he might be a kitty that can be controlled by diet alone. Would you consider setting up a spreadsheet like we use here so we can better offer advice?
 
Thank goodness you are home testing! Imagine the folks that don't test and just take orders from the vet. I agree with you by lowering the dose in half. Check out the sticky in the Lantus/Levemir forum about the Start Low Go Slow (SLGS) method.
 
I should have one in my signature, "TK's SS", though I keep getting error messages when I try to update it - the signature that is.
I must be blind! I see it now. :confused: Are you following one of the two Lantus protocols described in the info stickies on that forum?
 
We're basically on the SLGS, just didn't start low enough. The rest of the protocol is very close to what my vet outlined.
Looks like you can stay with 0.5 u for now but lower it by 0.25 u if you get 90 or below as a BG according to SLGS protocol.
