I think you could really start waiting for a +2.....+1.5 is really pretty early for onset on Lantus and in some cats, we don't see it until +3

When Ozzy went low on the Vetsulin, did he come looking for food?

(don't forget to change your signature to indicate you're now on lantus)

Oh no....maybe I misunderstood. When/how often should I be checking? Maybe I'm ear poke happy now! OMG..my poor kitty!!!

Yes, I can wait until +4. My sweet Ozzy is all snuggly on my bed for now. I still have PTSD from Vestulin!

Ok, sounds like a plan. I know his 'depot has not filled up yet" but his last two (and first two) cycles on lantus, he actually dropped at +1.

When Ozzy went low on the Vetsulin, did he come looking for food?
Gosh, I feel awful saying I don't really know. I have a fattie kitty who loves food. He is always spocking the kitchen for something. The two times when he went into the low greens, he did not come for food. I only knew because I was monitoring and brought food to him. In retrospect, I should have got him up and made him go to the kitchen so he knows to go there when he feels like that.

Marje- here's some more info: I checked my work schedule for Tuesday (Damn....18 days of vacation went by in a blink of an eye!!!:() and I can be at work by 9am. I am an hour away so have to leave no later than 8am. Need to get my shot schedule backed up till then. Hubby's start time Tues is still in the air.

I have actually been reading a lot of your posts about Punkin (one of Ozzy's nicknames is actually Pumpkin!!) and was so sad when I learned he had GA. He looks like he was a big cuddly sweetie.

I notice you mentioned he had acromegaly. My vet was concerned for this for Ozzy, too. They actually put it on his DX when we left the hospital which kind of pissed me off because we declined to do the blood test to confirm. We had asked what the tx was for acromegaly and they said surgery or radiation. We both decided then and there that we would not put our kitty through either of those treatments (though I know others have the courage to do so- bless them for their strength) so we declined the blood test at the moment. We wanted to see how Ozzy would do on insulin and deep down I feel his DM is more related to the fact that he was an 18-19lb maine coon (IOW a fattie) rather than he had a rare pituitary tumor (though I learned later it is not that rare). We will see how he does on insulin. He does not have the physical characteristics of acro and has been responding well (sometimes too well) to insulin so far.

So at what time did you end up shooting tonight...9? If you shoot at 8:30 tomorrow morning and 7:45 tomorrow night, you could shoot at 7:30 Tuesday morning and 7 Tuesday night.

I'm trying to decide on dosing for him. What complicates it is we don't exactly how much insulin he was getting with the 1/2cc and how big the drop was that you were squeezing off from 1u.

SLGS would have us leave this dose for seven days but if he's throwing even trace ketones, I think you should raise the dose to 1u tomorrow if he stays high tonight and as long as you can test if you raise the dose. He was seeing a bit of a response at 1u of Vetsulin with some green.

So the dose tomorrow morning depends on:
  • Does he have ketones
  • Is he high and flat (pink or very close to it)
I'll be up before you shoot so I'll check in to see what he does tonight and post some additional thoughts.

Several of the acro cats here have very successfully received SRT (radiation) and done well in that their doses came down and they saw some pretty good BGs.

It's rather jumping the gun for the vet to think he's acro as we usually don't recommend testing for it and IAA until kitty gets to 6u bid on the dose. It is true that it is not as rare as thought; 1 in 4 diabetic cats are likely acro. It's not something you should worry about now but do know that a good treatment option does exist which can buy some good quality time.

Yes, we had Punkin and Anya together - they were a bonded pair. She is probably at least a little bit Norwegian Forest cat. She trills most of the time, although she can meow. She's now nearly 8 years old and is a bit of a cranky old lady! There are several pics of them in Punkin's profile that is linked in my signature block.

Punkin passed away in May 2013. We still miss him - he was an extraordinary cat and very people-oriented. My profile pic doesn't do him justice - he was a magnificent cat with his gorgeous long hair and fluffy tail.

In August 2013 we got 2 male kittens. Frodo is an orange long-hair. His body looks just like punkin, but his face is very different. Kind of pointy, triangular face. He is in love with Anya but she won't have anything to do with him. Sometimes he gets into hunting mode and she is his prey - it's awful. They get in fights here and there. I've got 2 feliway plug-ins going to try to help with it.

We also have McGee, a tuxedo short-haired boy. He's really sweet and gentle. Anya doesn't want anything to do with either one, but she'll at least tolerate McGee. I should add some pics of these guys to the profile.

Back to Ozzy - we don't even suggest acromegaly until a cat gets to 6u in dose. The growth hormones that are generated by the pituitary tumor act to basically block the entrance to the cells, keeping out the glucose. So glucose floats about in the bloodstream instead of getting into the cells. Acro cats are typically ravenous and have insulin resistance, which causes the dose to get bigger than average. There are many big cats - and I think I've seen 18lbs as an average for a maine coon - that don't have acromegaly. I wouldn't worry about this yet. Typically we don't see physical changes until later on, so I doubt your vet could look at Ozzy and diagnosis it. Let that worry go - one step at a time in this sugar dance!

Have you tested again yet? One thing I was taught when I joined was to put the time I tested/fed/shot (tfs) somewhere on my spreadsheet. I did it in the comments (you can look at one of my SS to see), but some people add a column before or after the dose amount to enter the time. It helps you and it helps us see where you are. I am in CA, too, so it would be more useful to me than to someone on the East Coast, of course.

Just FYI, I am a night owl and usually on the Board late (like till 2 am or so Pacific Time), so if you need anything at night when it seems like everyone else has gone to sleep, you can PM me if I'm not already posting on your thread. Our East Coast members are usually online by 3:00 am our time, and we have some European and Australian members that are around in between, too, so the board is usually covered 24/7. This week or so is a bit off because of the holidays.

Here's something else you might find useful: a "dictionary" of the terms we throw around here. We have our own "language" and it can be confusing to newcomers sometimes. One thing I want to stress to you is that you should never be afraid to ask any question. We were all new once, and we understand how overwhelming this can be. The only dumb question is the one you don't ask. We consider ourselves a family, and we take that very seriously.

I shot at 8:52 AM. Based on your guidance above, I will try to shoot at 8:30 tomorrow AM. 7:30A Tues Am should be doable.

I tested him for ketones again tonight and the result looked negative. So happy about that! Will continue to test him again tomorrow.

I can test all throughout his AM cycle tomorrow and probably up to +4 for the PM cycle. I have to get ready to go back to work Tues after 18 days off, so I really don't want to stay up too late.

So far tonight, he went up at +1.15 Pink) and then came down slightly at +4. We have the alarm set for +6 but I'm not making any promises that we won't sleep through it.

How long does it take for a dose increase to show up in the data? I read somewhere it could take up to 3days/6 cycles?

That's correct. It varies by cat (there's that ECID again), but typically the depot should be up to speed by the 6th cycle. That doesn't mean a cat can't show a change in the first cycle on the new dose - it just means don't expect it.
 
I see where Marje has a ketone meter she is going to send. Wonderful! The cheapest place I found the stripes were at Universal Pharmacy in Canada 866-456-2456 . It will take at least a week to 10 days to receive them .

Sorry...I overslept but we're ok. If you can post his AMPS here a bit early, we can discuss it. Since ketones are negative, I don't necessarily think we should rush the dose.

I think Tricia accidentally confused you. You don't need to put the test times in evey block. She just meant the AMPS and PMPS test times. I think that is up to you whether you want to do that. I did it the first few months or so but then stopped and it doesn't really affect how I give advice. It can be helpful if a member is not always able to shoot on schedule. You can just put it in the Remarks for the days you don't shoot in schedule.

Don't worry about taking the times out of the cells that you put them in.....it's no problem but you don't need to add them to the others.

How much did you get it for from the Canadian Pharmacy? I'm really impulsive and sometimes don't have patience (can't you tell by reading some of my earlier threads- HA!:woot:) and when I make up my mind that I want/need something, I want it right then. I did not want to wait to get the lantus in the mail and I was haggling my vet to get a script so I could start Ozzy on the lantus while I had a few more days at home to monitor before going back to work. This kitty is going to teach me some patience one way or another...because he leads in this dance.

Can you send a pic of how your lantus is packaged for refrigerated shipping when you get it in the mail.

Thanks also for watching us along the way....hope you are taking notes!!! Hehehe!!! Aren't the folks on this board AMAZING!!! I feel so blessed! I wish our Ms. Mogs would drop in everyone once in awhile. I am about to PM her a funny message with a picture.

Oh, Jo, impulsive often describes me too...if one is good, 10 is great , right !!! I went ahead and purchased 5 just for safety sake. I love me a bunch of backups...gives me peace of mind. And oh , my , yes, this is teaching me MUCH needed patience. I love order and rhythm to things and this experience is stretching me hugely in that area ...but it's good...reminds me of when my kids were babies, each day you could wake up to a whole new world !! You and Ozzy doing great !!!
