How long for blood sugar to go down with diet alone?

Hello... Wow this is all so stressful. I am a little on information overload and underload if that makes sense..

We put LB on low carb diet on friday. How long have others seen it take for the numbers to decrease on low carb diet? He is not on insulin yet... wanted to get an idea of his numbers while he was low carb. How long would be safe to wait? The vet said she is comfortable waiting a little while to see what his numbers do. And of course I have my fingers crossed that maybe he wont need it.

And since he became low carb have not seen him have a bowel movement since Saturday...Today is tuesday.
Along with our other cat who is now also low carb..

Usually about a week is long enough

You don't want to wait too long to start insulin because the sooner you get them into normal numbers, the better their chances of having the pancreas heal and resume it's job!
 
My Spitzer dropped about 200 mg/dL switching to low carb, within a couple of days.

My Rain dropped 100-200 points within a couple of days as well (you can see the transition during the weekend of 5/6-5/9 on her spreadsheet in my signature - she was suffering a pancreatitis flare at the time, stopped eating completely, then went straight from only eating dry to only eating wet).

If LB hasn't had a bowel movement since Saturday, I'd be concerned about constipation. I would suggest giving your vet a call, and possibly searching on this forum for constipation (I know several members have to deal with it on an ongoing basis with their cats but I can't think of names off the top of my head.

Are you testing blood glucose at home? If not, and the blood glucose is being checked at your vet, please be aware that stress can significantly raise the blood glucose (my non-diabetic cat Kaleigh routinely tested around 350 on her annual bloodwork, but in the 70s-90s at home).
 
I wish I had started insulin sooner --we tried diet alone for about a month which probably set us back for 6 months because he was probably pre diabetic for 6 months before DX-
All that to say if I could do it over we would have started insulin the day we were DX-
Miralax 1/8tsp 2x a day for constipation works well-- you can increase but that is a good start point. Make sure kitty gets enough fluids especially with miralax.

I am going to take his Blood sugar today (yes have at home pet monitor) and if its not dropped much by tomorrow we have a vert appointment on Saturday so will discuss starting insulin then. I appreciate the input as I really had no idea how long it would take to see numbers drop. I am leaning towards starting insulin soon. He did have a urine test done in January prior to this one in June so at least we know 6 months ago it was not at this point. But I do wonder if he has been struggling on and off for a while.

If possible try to get a reading when LB has not had anything to eat for a few hours...the longer the better to try to get as close to a fasting test as possible. Then if you could get a test 2 or 3 hours after eating that will give a good idea how well LB handles food without any external insulin. Readings can go up and down during the day even with a kitty that is not diabetic.

Thanks so much. That is a good idea. He just tested at 384 2 hours post eating his fancy feast classic.. I will make sure he does not eat for 5 - 6 hours and take test.
 
Did your vet discuss what type of insulin he might want to use?

There are lots of different types and not all of them are great for cats.....the three we recommend are Lantus, Levemir and ProZinc....they're long lasting and work the best in cats. A lot of vets will want to use Caninsulin/Vetsulin because they're used to using them with dogs but they aren't good for cats due to their higher metabolism

Now you will need to be prepared though....they aren't cheap!! In the US, Lantus and Levemir will cost about $300 for a vial and about $500 for a 5 pack of pens ....but a vial will last 4-6 months and the pens will last to the last drop (so they're the better buy...with the vials, you usually end up throwing about half away)

Now most of us are buying our Lantus from Canada which will save you a LOT of money!! All the information on where we're all buying is in this "Insulin from Canadian Pharmacies" post
 
Chris and China.. The Vet wants to use Lantus.. The plan is she is going to place order with our pharmacy so we can pick up on friday and bring to appt on Saturday morning to go over. If his levels are lower we can cancel appointment but that way we don't waste any time with appointment and then picking up and then another appointment. . I had not thought about the pen/vial comparison. Hmm I will go with whatever she orders right now and if he still needs in future will look into purchasing from Canada. Thanks for the link!

When you give Lantus at first what do you suggest for making sure you are home to monitor. I am planning sat morning so we can be home though in and out over weekend.. little more concerned about work week.

While you have a day to look into it, you might want to call around and see if you can find a pharmacy that will let you split open a box of pens and buy just one....A lot of vets don't know about the pens, but they're the exact same drug, just in a different form.....each pen holds 300 units and you still use regular insulin syringes to pull the insulin out of the pen. We don't use the special needles designed to go with the pens (they are for humans to use)

Target stores, hospital pharmacies, some Costco pharmacies are some places we've heard that might be more willing to sell just one pen.....300 units is enough to last most cats about 3 months (or more..depends on dose).

One pen will cost you around $80-$100 instead of spending $300 on a vial that you'll most likely end up throwing half away

There's all kinds of great information about Lantus in the "stickys" at the top of the Lantus Forum. The most important thing to learn is that it take 5-7 days when you first start before you'll know how well your cat will respond to the dose he's started on. It takes that long to build the "depot" that lantus requires (it's kind of like timed-release...out of each shot, some goes to work and the rest goes "into the depot" and is slowly released back into the bloodstream)

We start most cats here on somewhere 1 unit twice a day so if your vet suggests a higher dose, I'd really recommend you post and ask for advise here first. Most vets start their cats at way too high a dose! (My own vet suggested 4 units as a starting dose for China...at the time she weighed less than 8lbs and in the 4 years we've been doing this, she's never needed more than 2.75 so I would have seriously overdosed her if I'd listened to the vet)

It's much safer to start lower and slowly go up so you not only keep kitty safe, but also don't miss out on what could be a "perfect" dose. We only do increases in .25 unit increments here.

Because Lantus does usually take several days before you start to see real results, you could probably safely start on Friday night (if your vet doesn't go ahead and start on Friday morning....but remember, the shots need to be 12 hours apart). A few cats will respond sooner, but generally, it's going to take a few days before you'll see what the dose is truly going to do.

If your vet doesn't know about the pens, here's what we're talking about.....Again, you'll still need to get regular insulin syringes to use with it (or with a vial).

You'll want to get 3/10ml, 30 or 31 gauge insulin syringes with half unit markings

They're also available at WalMart if you have one near you..Their Relion Brand 3/10ml, 3o or 31 gauge, 8mm insulin syringes all come with half unit markings and are only $12.58 for a box of 100 (although it seems a lot of WalMart employees don't realize that they DO come with half unit markings!!)

Wow.. thanks for all that information!!! I just took LBs blood sugar after him not eating for 4 hours. I learned 2 things.. He is very strong!! We had quite the situation getting some blood.. His number was 308.. lowest I have seen. Will try doing blood sugar again tomorrow morning after longer fast.

Very good information on starting dosage. If we go insulin route I will indicate to vert we would like to take a conservative approach.

Good to know about pen.. Not sure I am going to have time to seek out the pens.. but will keep in mind and ask at pharmacy
 
Here's something I wrote up for others that needed help with testing...maybe it'll help you too!

It can be really helpful to establish a routine with testing. Pick one spot that you want your "testing spot" to be (I like the kitchen counter because it's got good light and it's at a good height....it also already blocked 2 escape routes due to the wall and the backsplash) It can be anywhere though...a rug on the floor, a table, a particular spot on the couch...wherever is good for you. Take him there as many times a day as you can and just give his ears a quick rub and then he gets a yummy (low carb) treat. Most cats aren't objecting so much with the poking..it's the fooling with their ears they don't like, but once they're desensitized to it and learn to associate a certain place with the treats, they usually start to come when they're called! Or even when they hear us opening the test kit!

You also have to remember...you're not poking him to hurt him...you're testing him to keep him safe and understand what's going on inside his body. There's just nothing better than truly understanding what's going on inside your kitty's body and with this disease, the more knowledge you have, the more power you have against it. The edges of the ears have very few pain receptors, so it really doesn't hurt them. Also, if you're nervous and tense, it's going to make your kitty nervous and tense too. As silly as it might seem, try singing! It forces you to use a different part of your brain!

It's also important to make sure his ear is warm. A small sock filled with a little rice and microwaved or a small pill bottle filled with warm water (check temp against your wrist like you would a baby bottle) works well.

When you're first starting, it's also important to use a lower gauge lancet, like 25-28 gauge. Most of the "lancet devices" come with 33 gauge lancets and they are just too tiny to start with. The bigger lancets (that are lower numbers) make a bigger "hole". As you poke more and more, the ears will grow new capillaries and will be easier and easier to get blood from...we call it "learning to bleed"

Finding the right "treat" will be a great help too! Freeze dried chicken, bonito flakes, little pieces of baked chicken...whatever low carb treat you can find that he really enjoys will help him to associate the testing with the treat! China's Achilles heel was baked chicken, so I'd bake a piece, chop it into bite sized pieces, put some in the refrigerator and freeze the rest to use as needed. It didn't take long for her to come any time I picked up the meter!
