BMJ; Sorry: family issues here, and a late reply. Stubbie's numbers on Lantus 3.5 2X day were still around 300, high 200s at nadir, then down to 70 one night pre-shot so I did not shoot. Up to 300s the next morning. Very confusing.

I have just switched to ProZinc. He was 300 on 2 units at 5 hours today. He's had 3 shots total since we switched.

1) Should I raise to 2.5 now?
2) Is there a chance this cat is just not able to be in the 100s? I feel these high numbers are damaging him. He already has cataracts.

I will keep a spreadsheet updated as I will be home now for a week.

In cats, cataracts from diabetes is actually pretty rare. Usually it's genetic or an injury of some type. Has the new vet had a chance to look at his eyes yet?

No, I haven't seen her yet due to work. Soon!
It is hard to advise without seeing several days of data to get a feel for what is happening.

Have you read over the ProZinc protocol?.

A really important point: insulin doses are adjusted based on the nadir, the lowest glucose post-shot. They are NOT increased based on the pre-shot levels. The nadir for ProZinc is usually between +5 to +7 hours after the shot.

So I am not doing this correctly. I cannot test this cat every hour, or every two. I try to find his nadir at 5 - 7, yes, and it was high 200s or 300s on 3 IUX2 of Lantus for maybe a year. Soooo, I "thought," after testing him PreS one night last week, and him being at 70, that I would kill him with another shot. Hypo.

Should I have stuck with the Lantus? Am I "starting over" now with ProZinc and am going to need to test him several days in a row?

My former vet said not to test him so fervently, and to try and catch the nadir, due to my work.
 
You don't need to test the cat every hour.

Test at pre-shot to make sure it is safe; for new insulin or new members, 200 mg/dL is the cut off until you have some data to show it is safe to shoot lower.

When you can, to see how the insulin is working, try to get nadir period test. Weekend, a day off, or if the schedule is really screwy, set the alarm for some time at night.

Nadirs do vary day to day; the idea is to make sure he isn't going too low (below 50 mg/dL on a human meter; 68 mg/dL on an AlphaTrak)

I'm testing him every 2 to 3 hours today, and he is already bouncing again. Without being doom and gloom, this has been going on a long time. He went from 212 to 370 in an hour today, and he
didn't get into any carb food. It blows my mind, and this is what happened with the Lantus--so it's not the insulin (darn it). My heart breaks because this can't be good for his body. I did shoot 2.5 this morning,
since the 2 units wasn't doing its job. I also don't totally understand the preshot number: for example, the other day he was 70 at night PS, and I was not going to shoot then. And of course, the next
morning, 370. Is there a chanc he is not "fixable" due to many years of being uncontrolled before I adopted him? He was a mess, honestly, and did not get the care he needed until a saint from DCIN stepped in...I flew him to me
from Illinois, and he is such a baby teddy bear. I want to help him.
