Is Remission really over?

Hi,
Trina has been in remission for a year. She was a previous diabetic for 8 years. I know it's not the norm but she has always done great on 1 shot a day (a small amount of ProZinc in the morning). I had full panel bloodwork done on her about a month ago on Dec. 30 & the glucose was 160. Yesterday afternoon I noticed her going in & out of her litter box & only a drop or 2 would come out. When I caught some urine in a container it was tainted red with a fleck of blood. I took her to the ER & they said her glucose was 510 & she has an infection. They sent the urine culture out to identify the exact type of bacteria. Dr. said she has alot of inflammation, I think in the bladder area. She gave her a shot of antibiotics & sent me home with a week of Clavamox. She said to follow up regarding the diabetes with my regular doctor. She also asked if Trina is vomiting or drinking excessive water & she is not. Trina did have a slightly increased appetite recently & she gained a pound. I'm not seeing the typical signs of high glucose but it's showing in her blood & in her urine. So I just called my regular vet & she told me to bring her in next week when the antibiotics are done along with a urine sample. I asked the receptionist if she was sure the Dr. didn't want to see her today & get her started on insulin so she asked the Dr. again & she gave her the same answer with no explanation as to why. I'm not very happy about this. The receptionist said they were swamped with Monday morning phone calls & emergencies coming in so maybe that's why she didn't go into detail but I'm planning on calling back later. I'm very concerned about her glucose staying this high for another week. Does that sound normal to let her go a week with high glucose??? Would there be a reason not to start her on insulin ASAP? I wonder if the Dr. is thinking the infection would be causing the glucose to be out of whack & maybe she's not going to need insulin again. I have no idea! Trina is usually on a strict diet of wet food only but yesterday she ate a small piece of bacon. I wonder if that caused the pancreas or bladder to flare up. As you can see I"m grasping at straws trying to find another reason for the sudden high glucose. Thank you, Missy & Trina

I'd just keep up the antibiotics and watch her closely. I can completely understand the worry though. I just realized the other day that in 1 month Hidey will be in remission for a year already! Time flies! I know I start to get jumpy when his number rises a little bit and start watching closely. Trina's numbers are most likely high do to the infection, only time will tell. I'd give it a few days for the antibiotics to do there thing and if the numbers stay high then intervene with insulin. Theres no telling if the infection is causing it or something else. I'd be worried if her bg was raised due to the infection you'd have more of a chance of a hypo episode simply for the fact that the antibiotics would start to work, and maybe kick the pancreas into overdrive. I am not a vet though, and luckily the only issue I've had with Hidey's bg being high has been diet, weight, and bad teeth.

Urinary infections or any infection can raise blood glucose and so I wouldn't panic just yet. I would start monitoring her blood glucose with a meter again and try to get a few readings each day. So say before her breakfast, etc. that way when you go to the vets on Monday you can have a discussion about the last week. I would also be tempted to get some ketostix to test her urine for ketones.

My cat remi had a bad pancreatitis flair after he licked some cold sausage fat but I he was visibly unwell. Make sure she is eating and drinking well.

Thank you for sharing your thoughts!! I sure am hoping that the high numbers are due to an infection & not diabetes returning. And I hope Hidey stays in remission for a long time :)

OMG I can't believe Remi had a pancreatitis flair from licking cold sausage! Yikes! I do have some ketostix. Thanks for the advice!

Technically the diabetes never goes away; OTJ cats are diet-controlled diabetics.

I'm in a similar situation to yourself at the moment. Saoirse has pancreatitis (and maybe IBD). I got a new batch of her regular food last week and it triggered off her pancreatitis symptoms again and her BG went out of regulation for a few days. I've switched her to another food and gone back to giving her ondansetron BID for the nausea. Thankfully her BG is getting back to regulated numbers but I will be monitoring her daily again until she stabilizes.

Fingers and paws crossed that Trina's numbers will improve as the infection subsides.

It wasn't the sausage it was the fat that had come out of the sausage. So it was pure fat. Nowadays I am ultra paranoid about leaving anything out after I cook :nailbiting:
