Sorry to hear the 2.0 units didn't work for Milo. Hope you get some answers from the experts before you have to shoot tonight.
 
I'm sorry Milo seems to be bouncing again today. I'm having a hard time coming up with a recommendation for tonight's dose, without enough test data. Milo has been on this dose for almost 4 cycles now, but you only have test data for two cycles. Due to the later nadir of Lev and no test data after onset the last couple of nights, I don't know what this dose is doing for him. Fast tracking is a more aggressive form of dosing, and with it, you do need to try to get at least one test each cycle. I wonder if you should stick with the same dose for tonight, and see if you can get a later test in. Maybe drink a large glass of water before bed and test him when you get up. I'll ask some others what they think.
 
The risk is that without enough test data, you could by-pass the optimal dose.
With that in mind, maybe compromise between the fast and regular track and hold another cycle, with some mid-cycle tests?

The protocol calls for .25U increases if the nadirs are over 200, but less than 300.
Increasing the dose:
  • Hold the dose for 3 - 5 days (6 - 10 consecutive cycles) if nadirs are less than 200 before increasing the dose by 0.25 unit.
  • After 3 days (6 consecutive cycles)... if nadirs are greater than 200, but less than 300 increase the dose by 0.25 unit.
  • After 3 days (6 consecutive cycles)... if nadirs are greater than 300 increase the dose by 0.5 unit.
