? Wont eat, 394, insulin?

Sprocket wont eat this morning. No pill pocket. I syrnged some famotidine into him. He had a little tuna juice. Had a few kernals of zero dry. What do I do with insulin? He is high. 394. 383 last night before shot. He is licking his lips and moved his water dish all over last night n drank alot.
What do i do?he has vet appt at 1030.
 
Those are signs of nausea. If the famotidine does not help, then he may need some anti-nausea meds such as cerenia. Was Sprocket tested for pancreatitis? If he tests positive for pancreatitis then he will need some anti-nausea meds, pain relief such as buprenorphine and maybe some Sub Q fluids. An appetite stimulant could also be used, but is of little use as long as he is feeling nauseous.


ETA If need be syringe some food into him so that you can give some insulin. With a previous DKA he needs to have some insulin on board.

I have cerenia and appetite stim but he wont take pickpocket to take them. I have been doing fluids daily for 10 days already. Idk if test for pancreatitis itseld but tests point to no a few weekz ago. He has cholangiohepatitis. He was in hospital beginning of month.

I also replied on FB. With that DKA he needs some insulin. Since Lantus "usually" has a nadir at +4-+6 you can still give insulin even if a bit reduced, and he will be at the vets by the time there should be any real movement in glucose levels. Have you checked for ketones?


ETA With liver problems it is important to get some food into him. Can you syringe feed him some food? When I have had to syringe my kitty I used the Hills a/d Urgent Care since it is very easy to syringe. You might want to pick up a couple of cans when you are at the vets to keep on hand.
 
If you could get a couple of ounces at least that would be a start. I am scared to give a dose suggestion. If it were my kitty....with a history of DKA, numbers in the flat higher range and the fact that he will be at the vets in 3 hours I would be tempted to give the full dose. My reasoning is that keeping the DKA is important and with the onset of Lantus usually at +2 or later and nadir usually between +4-+6 or later he will already be at the vets by then. Again this is just my opinion and there are not a lot of people on right now to add to this.

I agree with Mary Ann about the need for insulin. I'm not sure about the dose but the reasoning for a full dose makes sense to me. I'm glad Sprocket is going to the vet today because he's still vulnerable after his DKA earlier in the month. If pill pockets won't work I'd try hard to give the meds manually or with a pill gun. It's a good tool to have if you don't have one.
 
Please make sure your vet tests for ketones when you get there. Also cerenia can be given as an injection. Some say that it does not work as well as an injection, but it has certainly helped my kitty when he has had pancreatitis flares. My vet gave (sold) me some prefilled syringes with cerenia and buprenorphine so I could give the shots at home, especially if he wasn't eating or I couldn't pill him

At vet now. Syringed 1 tsp of wet food watered down. Gave 1.5 of insulin. Usually give 2.5 units. He is talking which he does. I brought some dry food incase he eats something. The zerocarb stuff. Will let u know what happens. I will test for ketones and probably alot more.
 
Ketones. At ER vet now. Waiting hour plus here. I guess ketones isn't important enough to be seen right away. Omg getting annoyed!!! He is restless. Has had no water or food.
 
So sorry Sprocket is throwing ketones again. How high was the reading? If it is high they should not be making him wait. I'm glad you gave the shot before you went. With ketones you definitely want to keep insulin on board.

3. -Electrolyte imbalances need to be corrected" They should not be waiting to start the treatment.
 
This is maddening!!!!! It's an ER vet, they should be good at triage if nothing else!!!! :banghead::banghead::banghead:

Keeping fingers crossed that Sprocket can get the treatment he needs asap.
 
His triage was a few questions. I had to ask for a room. He had been in cage 2 hrs already. I had to ask for a water dish. Usually this place is great. Never seen it that busy plus it was so many dogs.

He is overnight with iv fluids and gave insulin 3.5 units after 397 of bg on pet meter.

They want him to eat DM. I told them he won't stay with one kind of food. I have him on fancy feast pates n if needed zero carb dry with some evo dry. Low carb.

She is gonna look up zero food. Frustrated. So sick of vets knocking this site without even looking at it.

She thinks I am micromanaging too much.
