We have been keeping track of Lucifer's AMPS and PMPS. A few times we have done the 6 hour mark, and in the first few days of beginning all this we did quite a few checks. The vet initially gave us a ridiculous dose of 3 units once a day, and then 2 units twice a day. In those first few days his levels actually dropped into the 100's but we backed off with his dosing to 1 unit 2x a day to start lower because he dropped and we actually had to skip a dose. So now that it has been over a week at the same dose we are doing our curve and sending to the vet tomorrow. However he seems to range in the upper 400's and low 500's with this dose. Does that mean he will most likely need an increase? I hate the needles we have they go by two's and it has been awful. We are getting the ones suggested on here in the mail soon. Lucifer has gone back to being himself and eating the wet food so thats a big plus. Now I just have to figure out the best dose to get him back in the 100's and low numbers.
 
It does look like he needs an increase. Your spreadsheet says TR, but signature does not say - are.you following TR? If that is the case (1) you do not need to wait so long between increases at these high numbers, you can increase as soon as every 6 cycles (after making sure any bounce has cleared), (2) please be sure to get one additional test per cycle aside from the PS at varying times, and (3) there is often no need to do a curve if you are doing the testing in #2. But in this case yes, go ahead. I don't expect it to tell you much other than he's high unfortunately. You would also probably want to do a a curve if, while following TR, you are limited in the extra tests you can get.

If you are following TR and his curve today stays above 300, you can do a 0.5U increase.

Edit - now that I found your old post I see the guidance was to wait 5-7 days for depot to build/stabilize which is correct. Since he's been pretty high you could have increased closer to the 5 days with TR, but no harm no foul because the extra tests we look for aren't there (not a criticism, just explaining rationale)

Yes I am. I must have missed that when I initially did my signature. I will add it straight away. Okay that is good to know. I can increase as soon as I get these new syringes. The vet gave us ones that go only by twos and it’s so hard to determine where 1 is even. It’s a struggle. But I have the 1/2 units coming thank goodness. Yes he dropped quickly in the beginning (too quickly) so I know he responds to the insulin but just is not responding with 1 at this time.

Part of what you're seeing is some glucose toxicity as well, their body kind of adjusts to the high numbers as normal and it can take a bit of increasing to break through it before they come back down in BG and dose - so don't be surprised if you actually end up going over 2U again. That can be confusing to a lot of people, just remember doses aren't static. Insulin is a hormone, cats' insulin needs change wax and wane over time

Our vet has us doing his curve this weekend to send in the results and it’s every two hours so I was trying to give him a slight break before then. We have the worst trouble getting him to bleed it’s awful.
 
I just tracked his syringes and they are coming Monday. I plan to keep filling through the online pharmacy so he gets the half unit ones and not these awful ones.

We recommend varying the times as you're able. So a lot of us start with a +2 or +3 as it usually indicates how a cycle may go (flat or drop means you'll probably need to test again, rise often means you're good to stop testing, just have to figure out his trends first). But other than that sort of a smattering - a +4 here, a +9 there, +6 there. Once he has a more noticeable nadir (low point) yes you'll want to try to test close to that, since we dose based upon nadir

Yes, most people also do the +2/+3 "before bed" test. A lot of people choose to shift their shot times to account for that and maximize sleep. Some people like 430/530 am and pm so the bulk of the action happens during the day. Just get up, test-feed-shoot, back to bed lol
 
To add to what Melissa has explained, cats often go lower at night so a +2-3 s really helpful. If lower than the amps/pmps it’s likely to be an active cycle and if a lot lower it’s good to set an alarm for a test at +6.
If you intend to follow TR, you need another test each cycle besides the preshots every day. He either needs an increase, (most likely), or he’s bouncing from dropping bg later in the cycle. I’d suggest getting those extra tests so that when the syringes arrive on Monday we know what the next dose should be.
 
Sounds good. I can test a couple hours after morning shot and a couple hours after evening on Monday that way we know where he is at a bit better.
