Cassis not himself for last two days. Not sleeping and not taking syringe feedings without a struggle. No bowel movement for two days on Miralax. Insulin requirement dropping as well as his nadirs. Scary bout of low numbers today at mid-cycle. Saw the vet afterwards, who found him dehydrated and recommended resuming subcutaneous fluids, which we had stopped because of his presumed heart failure. Lowered his insulin to 1 unit tonight and he is 158 at 4 hours but just vomited before we could feed him. Don't know how much more he will drop. Could be a long night.
 
That doesn't sound good. Does the vet have some ideas about the constipation? Here is a website with some info:

You know what to do with low ranges, right? Feed with low carb first, then the gravy off high carb, then honey. I hope he doesn't drop too low.

I hope he starts to improve. Paws crossed he feels better soon.

Thanks much. His 6 hour is up to 193, so it looks like we are out of the nadir and can relax a bit. Will try to get in a small low-carb feeding before bedtime.

Thanks. Troubles continue. Still not pooping and is mopey despite the fluid and Miralax. Sugar dropping at PMPS despite a low dose this AM. Looks like no insulin for him tonight. Waiting for vet to call back to discuss giving more fluid or something something more invasive like an enema. Constipation and dehydration seem to be the main concern now. Not sure whether they might tie in with the low blood sugar or if they are separate issues.
 
I hope the vet can figure out a way to get him to go - constipation can be bad. An enema, checking for an obstruction - both might be possibilities.
 
Rough day for Cassis. Saw the vet again today and had an enema and more fluid for dehydration. Blood sugar low for him (90s) all day despite only 0.5 units. Not sure if this is related to dehydration or, more likely, to poor eating. He has been too weak to do much of anything, but he does resist his syringe feedings. Vet says he should perk up with the fluids and enema, but so far not. Doubt that we will give him insulin tonight.
 
The only rapid option is an enema.
Pushing feces through the GI tract too quickly may cause tears in it, which can be fatal. Best to start with the vet and possibly an enema. then use regular (no pun intended!) preventive treatment using one of the options described on Feline Constipation

Thanks. The vet gave him an enema yesterday and he passed one firm piece and some liquid stool. His main problem now seems to be his dehydration. He has been getting subcutaneous fluid but is just urinating it out. To make matters worse, he is too weak to make it to the box most of the time. Also had a seizure last night (blood sugar 163 at the time), which may be the result of the dehydration and stress that he went through yesterday. Still lethargic today and has managed to swallow only a small amount of food by syringe. Blood sugar 196 this morning but no insulin given. Waiting to hear from vet. Not sure what the options are.
 
I wish I knew something more about this. Is your vet wondering about some kind of obstruction?

Maybe post on Health with his symptoms? It's amazing what the posters here have gone through and know about.
