tiffy 7~13~13 amps, 426 alpha, 284 micro 2.5 open

Hi this morning I had a big differential. Alpha 426 @amps and 284 micro. @ +5 it was only 50 so alpha 274 and micro 224. I want to do a few more. Now regardless her nadir is high for the numbers. In amps the unit was 2.5. Id like to raise it but to what? Now she is not being nadir friendly w the tests but I feel the few nadirs we have we can adjust her up .25?
 
Theresa, can you get your ss current? I'm reluctant to suggest an increase without seeing recent numbers. And i don't like the difference in meters, since it was much closer just a few hours ago. Wonder about a meter or blood issue......

Of course the 426 would merit an increase, but is that a typical amps for the last few days? The 288 could also merit a small increase, but again, wondering if either number is correct....
 
The alpha is correct but the difference is huge and sometimes it is 40, 50, now 144 difference? So if I only added 40 points on I wouldn't be dosing what she needs so for micro she needs more insulin w the lower number
 
1) We know the meters read differently.

2) The difference is greater at higher numbers because the +/- 20 % ranges gets wider
Ex
50 -> 40 to 60, a range of 20 mg/dL in which the "true value" lies
500 -> 400 to 600, a range of 200 mg/dL in which the "true value" lies

3) If 0.8 * the higher number is less than or equal to 1.2 * the lower number, the numbers may be considered the same.
0.8 * 426 = 340.8 vs 1.2 * 284 = 340.8
These 2 numbers may be considered the same.

4) Pick 1 meter to use and adjust your scale only with that meter

5) Use the reference numbers for that type of meter
.
6) Do not drive yourself crazy because one meter reads like a Celsius thermometer and 1 meter reads like a Fahrenheit thermometer.
 
The difference between the 2 meters won't be the same at every value in the meters.

I don't know if you ever had to graph things in high school or college algebra using any kind of exponent, but those kinds of graphs are curved - as value X increases, value Y increases exponentially. It looks to me like that is how human glucometer test values compare to pet-specific test values on the same sample of blood.

A very simple example using Y = X squared
When X = 1, Y = 1
When X = 2, Y = (2*2) = 4
When X = 3, Y = (3*3) = 9

We can figure out a more exact equation with Excel and lots of data pairs.

The important numbers are the low ones, when your cat could be heading towards a hypoglycemic range. Because that can be life-threatening, we need those numbers to be more accurate and they are.

okay, so it looks like 3s and 2.75s is what she needs with the blue in nadir, and that is consistent.
 
BJM, I talked to Theresa on the phone and she told me she is 'math challenged'. ;-)

We talked about the differences in the meters, the roughly 30% difference at the higher numbers. Theresa wants to do more testing before she is willing to switch over to the Micro completely. She needs that comfort level. She did say, she likes the lower cost of the test strips with the Relion Micro.

Theresa told me she can tell that Tiffy is feeling so much better than when she first took her out of the kill shelter. Tiffy has wormed her way into Theresa's heart and I don't think she will ever be put up for adoption, since she has found her furrever home!

We walked through her SS, pointing out examples where she has test data to help her decide on the dose. Also examples of where she does not have mid-cycle numbers on those 300's to know if the dose is correct or not. After walking through the SS, Theresa only saw two ranges of numbers she felt comfortable with increasing the dose and realized she needed more mid-cycle tests for some of the other ranges.

I told her to look at some of the AMPS and PMPS data and put her hand over the mid-cycle tests to block them out. If she didn't have that mid-cycle test, there would have been no way to realize the dose was dropping Tiffy into better numbers. I think that helped her to understand.

We looked back to the beginning of the SS and could see there are quite as many black numbers and not as high numbers when Tiffy does get that hi. She still hates to see any reds and blacks, wants to see those 300's and 200's at pre-shot and more blues and maybe even some greens at mid-cycle. I told her it will take more time. Look how far Tiffy has already come.

Theresa was thinking of trying these dose changes for 2 weeks and I suggested only 1 week and reevaluate at that point. She said ok.

I reinforced the idea of looking at the patterns over more than a single cycle to see what was going on. Theresa understood the logic in that.

Tiffy is stubborn about getting those ear pokes, so we came up with a plan to feed her a teaspoon or so of food to get her cooperation. The treats simply do not last long enough. :lol: :lol: :lol: Hopefully, this will let Theresa get more of those PM mid-cycle tests.

We talked about Tiffy's various health issues and Theresa knows there is a long ways to go to get Tiffy feeling her best. She is one loved kitty. cat_pet_icon I got to hear Tiffy purr over the phone!!!! That was fantastic and I was able to share in Theresa's joy of having Tiffy in her life.

Theresa is feeding Tiffy and her other 2 cats Dharma and ? (I'm blanking this AM ohmygod_smile ) near each other so that they get more used to being close to each other and start to feel more comfortable. Use of baby gates is helping to keep them separated. There is progress being seen there.
