Thoughts on these numbers?

So, I tested Duncan right before I gave him is breakfast and insulin at 8am and he was at 350.
I gave 1 unit of Lantus and now, a little over 4 hours later he is at 294 right now.

I need to go read more about the Lantus I think. I thought it should start peaking around 4 hours...? Too much information for one 24-hr period! :)
Maybe he just needs to be on it for more days to give it a chance to pick up and start working properly???

I thought I would see more control after 4 hours...

Thoughts?
 
If he got his first shot yesterday, he should be filling the shed:

So you may not see much movement today or you might see a little. You wil get thoroughly sick of this expression - ECID. It means Every Cat is Different.

This info from the Lantus stickies is a "typical" curve. But remember, that is after the shed is filled and he is using the insulin, not storing it.

Example of a typical Lantus curve:
+0 - PreShot number.
+1 - Usually higher than PreShot number because of the last shot wearing off. May see a food spike in this number.
+2 - Often similar to the PreShot number.
+3 - Lower than the PreShot number, onset has started.
+4 - Lower.
+5 - Lower.
+6 - Nadir/Peak (the lowest number of cycle).
+7 - Surf (hang around the nadir number).
+8 - Slight rise.
+9 - Slight rise.
+10 - Rising.
+11 - Rising (may dip around +10 or +11).
+12 - PreShot number.

Lantus is a little difficult to understand at first. Many people print off the stickies and refer back to them daily until they get the info into their brain.

Another question: are keytones in the urine the same as having sugar in the urine? The vet said there was sugar in his urine. I did notice that he smelled different to me on the day I took him in, but now he smells totally normal to me again. I can also tell he's feeling better. So, maybe not controlled yet but on it's way.

I keep thinking if I can just get through this first week or so without killing him he'll make it! :) Poor pincushion kitty.
 
This explains ketones much better than I could: ketones He'll forgive the pokes if he gets a treat each time.

You are doing great. Just think how much more you know now than you knew in those first few hours after his diagnosis.
 
So can a cat with pretty good numbers still have DKA? Or do you always see high BG when keytones are present? Is it just an escalation of diabetes?

(I know there is probably not a perfectly correct answer to these questions, your opinions are appreciated.)

Usually you see a low dose of insulin, high numbers and loss of appetite. But then there is that ECID thing. Some cats get ketones with lower numbers. Just something worth keeping track of and pretty easy to do.

We encourage every diabetic to be tested once a week or so. Anytime someone gets high numbers, we up that to daily. If a cat has ever had ketones, more frequent tests are also wise.
