They want to change Gucci from prozinc to lantus

They told me when I went to fit his new libre 2 and they haven’t fitted it on probably it doesn’t work so frustrating do I now check him with his BG monitoring system in his ears . As it’s so much hassle going to the vets for the libre to not be fitted correctly!!
 
How safe is lantus and how can we keep him safe without testing? And how will the change effect him? How much time a day do I need to use the ears pricked? So many questions again
 
It’s very safe. Ear pricks are much more accurate. Can you set up a spreadsheet and signature please? Just add a couple weeks of BG on the spreadsheet.

Once it is in stock, either myself or Emma will call you and discuss further about exactly how to administer it and the monitoring required. In the meantime, I would be really grateful if you could order the pen that Sophie has recommended to facilitate in giving the insulin to Gucci

"Thank you for sending through Gucci's latest curves. How is he doing clinically? He seems to be fairly sensitive to Prozinc, even at 1IU but the onset is quick and short-lasting, resulting in some steep peaks/troughs in his glucose curves although we can't see from these at what times his owners have been giving his insulin. Based off of the data from his most recent curves, he would be a good candidate to change over to glargine insulin which has a longer, more basal onset of action and will hopefully stop the rapid fluctuations in his glucose with the aim to achieve better glycaemic control. We would recommend starting on 1IU BID of glargine insulin and with this we suggest using an insulin pen (e.g. the JuniorSTAR pen) to allow for better accuracy of dosing, as glargine insulin comes in 100IU/ml formulations (Prozinc is 40IU/ml). He will need to be monitored closely in the few days following transition over to glargine and therefore it is recommended that he have a FreeStyle Libre monitor placed in order for his owners to be able to keep a close eye on him at home.

Please do not hesitate to get in contact should you wish to discuss this further or if you have any questions or concerns and we would be grateful if you could keep us updated with his next set of glucose curves.
 
We use syringes with half unit markings and adjust in .25 increments. It is good to monitor closely the first few days testing every 3 hours. Lantus is a depot insulin and it takes 5-7 days for the depot to build to see if the starting dose is a good one. You can read about the dosing methods we use here and see what you think. If feeding any dry food look at SLGS. If feeding a low carb wet or raw diet you can also look at TR. Since your cat has already been on Prozinc the starting dose would be the same for both methods as we take the previous dose into consideration. That’s why the spreadsheet and signature are helpful.

The freestyle libre that you have just fitted is not working so we don’t know what to do about this i will have test him using to BG strips it’s saying its not fitted properly and has cost us £50 for the system so can you please advise on this is the second time and it has cost £100 now can you please let me know what we can do to get a new one before the change of insulin.

They want to start on 1u as you can see on the post below
 
We would need to see the BG for at least a week or two to give opinions on dosing. Often the same dose given while on a different insulin is fine but sometimes if it’s bringing the cat too low a lower dose might be suggested. We are very data oriented here which is why I keep brining up a spreadsheet.
 
He was very high 559 (31) 10 hours after his 2u of prozinc. He was 531(29) last night. He had a trace of ketones in his wee. But was clear when I checked him for ketones 2 hours after his 2u of prozinc. Is 2u of prozinc enough or does he need to up his insulin?
