Tarragon - Pancreatitis Diagnosis - Thoughts?

We usually post on the Lantus board and only come over to health occassionally, so here is some background.

Tarragon is 11 years old next week. 6 years ago he was diagnosed with asthma. They gave him steroid shots whenever he would have a flare, which was about every 2 months. We kept asking if it would cause diabetes and they assured us
that he was not getting enough for that to be a concern. From Sept 2010 - Jan 2011, Tarragon lost 6 pounds, started drinking excessively, going outside of the lb, and not grooming. In Jan 2011, he was diagnosed with diabetes and has been
on Lantus since. When he was diagnosed with diabetes, we switched him to canned food. He had always been on dry. Since switching to canned, he has had liquipoo and is on flagyl (6 mg per day) long term. They have never said IBD, but I am assuming that is what they think it is.

That brings us to now. On Tuesday morning, I left for work. When my husband left a little later, he picked up Tarragon to give him a kiss and smelled what he thought was ammonia. (We now know it was acetone). I came home at lunch and
Tarragon acted tired and his BG was 186. I came home at 4 and he was unconscious and had vomited. Ran to the ER.

It took a day, but he was diagnosed with DKA (Diabetic Ketoacidosis) and acute pancreatitis. The first thing was to get him hydrated again with the iv. Yesterday, they started putting regular insulin in the drip to slowly reintroduce
insulin since he hasn't had it in 36 hours. They will start his Lantus back up this morning. Last night his ketones were at trace levels so they believe he will be released today.

What we have been told is that the pancreatitis was acute. His fPLi level was 9.4 ug. The pancreas wasn't as inflammed as they had seen. They are giving him small amounts of pain killer(bupe) to help with the pain. Last night I visited with the vet and she indicated that he would be sent home with bupe, anti-nausea meds, and sub q fluids.

I am really wondering about his food. Since the diarrhea started with the canned food, is it possible that he can't tolerate it? We have done the whole food dance thing, trying all of the prescription stuff as well as super market brands. Fancy Feast is the only one he will eat. We have been feeding classic for the low carbs. He is a picky eater when it comes to canned.

Is there any chance T has a food allergy? Zoe's tummy seems to be sensitive to Fish.....I was feeding her all the FF Classic Fish flavours, because that was all she would eat (she is extremely picky)....after a Pancreatitis flare in March, she had to go back on insulin in April....but she was still throwing high numbers and had liquipoo and vomiting. Our Vet suggested stopping fish as it is a common allergen in kitties....within a week of stopping all fish her vomiting and liquipoo came to a stop and her bg started coming down. It was hard to find kitty food that had no fish in it....even FF Tender Beef has fish!!! I think the FF flavors with no fish and are low carb are Turkey & Giblets, Chopped Grill and Beef & Chicken. I also found the Merrick's Before Grain foods that are fish and grain free and low carb. It took awhile to change her over, but now she does just fine.

For some cats, you may need to aim for lower fat content. I chose Fancy Feast Turkey and Giblets pate because it was more even between calories from fat and calories from protein while still being low carb.

Fat content in a meal has been associated with human and canine pancreatitis, not so much with feline; it might be helpful to go lower fat for some cats. You won't really know unless you try it and observe symptoms.

Sorry to hear of Tarragon's troubles, and glad to hear that he is coming home soon.
One of the best by-products of this board is that when things like this happen, it gives me a chance to learn more something everyday. I'd never really looked into pancreatitus much, because fortunately I haven't needed to. But there's always good old google.....
I just read about symptoms mostly, but this is from cat-world.com:

Pancreatitis can be acute (sudden onset) or chronic (persistent/long lasting). Cats with pancreatitis often exhibit vague/non specific signs. Both acute & chronic pancreatitis can range from mild to severe/life threatening.

Anorexia
•Lethargy
•Dehydration
The above three are the most common symptoms. Other less common symptoms include;
•Fever
•Abdominal pain
•Vomiting is commonly seen in dogs & humans but not a common symptom in cats.
•Diarrhea•Ataxia (loss of coordination)
•Weight loss

I'm just wondering if the pancreatitus might have been around longer, or maybe he has had previous acute attacks and they just happened to coincide with the diet change?

Carl - That is definitely my suspicion. I think it has been there lingering all along. It seems to bring everything full circle. He was fine on dry food but then the FD.

Sherry - I had the same thought as well. He only eats FF classic chicken and flaked chicken. I would really hate it if he was allergic to chicken! I cut out all fish thinking that may haven a problem. Plus he is susceptible to UTIs and then the mercury issue.
