Switching to Levemir Tomorrow Morning (+12 from now)

So life has taken away but also provided an opportunity to make the switch a day early since I will now not have to go into work tomorrow.

I increased the prozinc today, she bounced, and am assuming that that the nicer numbers I saw for 2 days were either a fluke or just how it's going to be with her.

A few questions that have come to mind.

1. When should I try to test tomorrow? I know you've said more frequently, are we talking every 2 hours, every 3....what would be ideal and then what is the window I really need to hit. I'll be home, but some days are good for testing her, some days she can get a little annoyed.
2. The evening PM dose, first time we'll have an overlap of action, same test times, more/less...depends on what we see with the first shot
3. Not that I think it'll be a problem, but no-shoot number since I'm having to use the Alpha Trek meter for now due to all her blacks. Guide says 200, will that work for the pet meter, or should it bump up higher?

Usually, when you first start a depot insulin, you're not going to see a lot of action from the insulin, but ECID so we do like people to be able to test more at first. I think if you test at AMPS, +3, +6,+9 and then PMPS and at least 1 "before bed" test, that's a great start. If you're a late night person and can get more tests on the PM cycle, it's always a plus!

you may not have any "overlap" until the depot is more full. Just get tests at different points of they cycle while you're learning how she's responding

If you get a Preshot you're not sure about shooting, you should Stall, Don't feed! and post for help. Test again in 20-30 minutes

As you gain more experience and data, that "no shot" level will start to come down. I did Tight Regulation with China and once I had enough data and truly understood how she responded to both food and insunlin I was willing to shoot lower and lower until I got to the point where I'd shoot anything over 50 (human) 68 on a pet meter.

Pink,finally! I think you could start Levemir at 3.5 units too.

Your goal for the next few days while you can test more, is to try to characterize Smudge’s reaction to Levemir, in other words, try to find her onset and nadir. Typical onset +4, typical nadir is +8. However, you won’t get a better idea of nadir until her overall numbers start to come down, and the depot is more full. The plan Chris listed for the first day is good, though I might get a +4 instead of +3.

It’s not usual to have nadirs around preshot with Levemir. You will have to forget what you learned about Vetsulin and Prozinc, and get used to shooting lower numbers. 200 would be a pretty high number to skip with Levemir, as the next shot won’t onset for several hours later, so her numbers will keep going up until then.
