Hello all, I haven't been on here in a couple of years after you helped me so much getting my cat, Freebie, to the type (Lantus) and dose of insulin he needed to be regulated. He is 14 years old, he's a good weight, happy, seems fine in every way, but his blood sugar readings have been too high for way too long, jumping up and down for several months. His usual dose was 3.0, then I gradually moved him to 3.5, and now I've got him at 4. I'm still getting readings in the 300-400's with an occasional drop to something like 40. I'm wondering if it's time to change to a different insulin and if so what would that be? He's been diabetic for about 5 years and was previously on Glargine. The Lantus worked well at first but really up and down over the past 6 months or so. Thank you for any advice you can offer.

What is happening (BGs) between shots?
Did you give the same dose every time like when you got the PS of 66?
Is there anything different that could explain the variations? Examples would be how much eaten and carbs is what was eaten.
 
I have been giving 4.0 during all these reading although the 41 scared me and I held off about an hour before giving the dose that time. Previous to this I was giving 3.5 and he was bouncing up and down.

He eats Fancy Feast Classic all the time, no dry at all, occasionally I will give a Friskies pate. I have 3 other cats, they all eat together, it would be impossible to separate the feeding from the other cats so they all eat the same. Sometimes the food is not all gone so I know he's not eating as much as normal but generally it is about the same. I take the reading just twice a day.
 
The 66>377 is likely due to bouncing, Giving 3.5/4 units at 66 likely resulted in going too low. Although Lantus should be consistently dosed, I would have given a significan;lty reduced does with a PS of 66, like one unit.

Have you not got any mid cycle readings? I can't advise on dose but I also think that it might be too high and some bouncing is going on. Could you get other readings during the cycle, even a +2 or +3 would help.

You need others to look at this. Can to amend the title of your post to away lantus dosing advice needed.
