How do I know if I'm tight or relaxed? I'm just starting lantus and coming off humulin. Chinookie weighs about 9lbs, so should we start with 2 units bid? That's the dosage we were currently on for humulin.
Starting a new insulin is overwhelming all over again, just when I got a little adjusted....

I'll post numbers later tonight, but please let me know if you'd advise 2 units based on his spreadsheet

Hi Erin! Welcome to Lantus Land! I don't have any advice for you (I'm currently a serious advice taker! :-D ) but someone should be getting back with you to answer questions at some point.

Hi Erin, welcome to LantusLand, the best place you never wanted to be.   I'm looking at Chinookie's spreadsheet and I'm a little confused. You have 4 dates in May but a note that says "Changed to lantus for pm shot". Is that tonight? Based on the 9 lb weight, Chinookie would get 1.0 u of Lantus. But typically, we advise to keep the same dose as the previous insulin. Do you have any more data on the humulin? I would also suggest getting a +1 and +2 test. Lantus dosing is based on the nadir (the lowest value in the cycle, somewhere in the middle) not the pre-shot value. So those mid-cycle tests are pretty important with Lantus.

Please ask all of you questions. We do our best to help.

Hi Erin and Welcome to Lantus Land!! I can't advise you on a starting dose, but to determine which protocol you want to follow is an individual decision that you have to make yourself. The TR protocol gives you the best chance of remission, but isn't for everyone since it takes a bit more testing and time to find "just that right dose"....but there are lots of us here to help advise you if you choose it!!

The "Start Low, Go Slow" is the other option. I'm not as familiar with it, but there's more information here. Start Low Go Slow

If you choose the SLGS, add that to your signature line so others will know that's how you're dosing.

The Tight Regulation protocol is here Tight Regulation

Good luck to you no matter which protocol you choose!! Even if you use the SLGS, you can continue to post here. We're a much more active group and you'll get more experienced eyes on your posts quicker than on the SLGS board
