Start insulin again?

Hi. It’s been a while since I’ve been here. My cat was diagnosed when she was 10 and then went into remission two months later after taking Lanthus. At 12, she was diagnosed with IBD based on symptoms(vomiting). We have found that Hill’s Gastrointestinal biome wet food keeps the IBD symptoms in check.I know it’s higher in carbs, but she can’t keep foods like Fancy Feast pate down. Other hydrolyzed food spike her blood sugar up to 400. For the past 3 years her blood glucose levels have been in the mid 300’s, and I know that’s not ideal. We recently brought her in for a check up and her ALT level was flagged at 326 in June. We just did a recheck and it’s 273. Our vet is on paternity leave until November so we spoke with a different vet who thinks she needs to go back on insulin (blood glucose 348 and in the urine, she’s been stable here).

I’m struggling with this because if she doesn’t eat several times a day, she dry heaves and we only could feed her twice a day when on insulin. He wasn’t concerned with the ALT but did express concern about getting back on insulin. I don’t know what to do. She’s 15 now, and I want to do what is best for her. She purrs, plays with our other cat and is social Any thoughts or insights are appreciated. Thank you!

I agree with the vet that your kitty does need to go back on insulin.
If her BG has been in the 300s I am surprised she has stayed well without insulin, she normal BGs for a cat are 50-120.
What insulin were you using before.?

She was on Lantus for two months until she went into remission. Thank you for your reply.
 
If I recall correctly, the “two meals a day for diabetic cats” advice is dated (even though my vet thinks it’s current advice). My cat also throws up on an empty stomach and needs regular meals and snacks throughout the day. It hasn’t disrupted his insulin regimen. I just make sure not to feed him two hours before his shot.
