Splitting doses?

Occasionally I need to be gone for the evening, and I was wondering if there was a safe/effective way to split doses, or if it would be better to delay and/or skip entirely. Pagan is currently on 2u of Lantus BID, and his usual insulin schedule is shots at 7AM and 7PM. So would it be okay to do something along the lines of usual 2u at 7AM, and then give him 1u at 4PM and 1u at 10PM?

Thanks!

Hi there, Liadh. I stopped in your condo yesterday, but didn't get a chance to say much, so I thought I'd drop by again today.
Splitting doses isn't really a good idea. Some folks on here are not able to dose on the 12/12 schedule all the time either. Cindy (& mousie) are one of them. Hopefully she will drop by and give you some input.

Splitting the dose plays havoc with the shed as well as with your shot schedule. If you shot at +9 (4:00) and at +15 (10:00), would your next shot be at 4:00 AM or 10:00 AM? If you wanted to shoot at 7:00 AM the next morning, you'd be shooting at PM +9 which would be a very early shot. Early shots act like a dose increase and you would be creating a large overlap with the previous shot. I think you are probably getting a sense of why this can be problematic. The easy answer is to either skip the shot unless you are home no more than 2 hours later than your usual shot time. However, if you shoot 2 hours late, you are still left with the problem of shooting 12 hours later in the morning which will, most likely, put you way off schedule.

Cindy/Mousie doesn't split doses -- she often has to shoot on an inexact 12/12 schedule due to work (e.g., she may shoot 13/11) and she shoots the same dose at each shot.

Because of my previous work schedule I used to have a 5AM/11 PM shooting schedule on the weekends. Normal schedule was about 4:15 AM/5 PM. Four of were on Lantus and it worked OK. I just had to expect a high 11 PM PS. Sometimes I would give a reduced dose at 11 PM since the next sot would only be about 5 hours later. I would rather go with a late shot than splitting a PM dose since splitting just further complicates things.

so you shot at 7am, and tonight you need to shoot at what time? And then in the morning at what time? What time do you HAVE to shoot in the morning to get out the door?

I would never shoot earlier than +10, and that is only under certain circumstances with guidance. I've seen +9 used once in a blue moon, but only with very careful monitoring and a very reduced dose. Larry's advice might work on occasion for a cat that is not tightly regulated, but Pagan has had some low numbers, and adding a very early shot on top of that could cause a lot of problems. Please post your specific situation and we'll try to help. Sometimes skipping is the best way to go, but let's look at it.

I was inquiring in general, but the specific situation is going out to dinner tomorrow night. I had planned on shooting at 7AM as usual tomorrow, but that is flexible as I have a haircut so going into work late. Not sure exactly what time I'll be going into work, but then my family is coming to town for dinner, so I'll be gone until at least 9PM. The next morning I should shoot as close to 7AM as possible, as I do need to go to work.

Try to be the most consistant with the shot as you can be!! Libby & Deb, Sienne, they really know their stuff when it comes to dosing, so I would try to follow thwm as best you can--OK?--Have a good time!

Liadh & Pagan, dropped thru & ready your condo today & wanted to say hi :-D
Good luck with the shot schedule & have tons of fun with family!!

yeah, just fwiw and all, i don't do split doses. personally, and this is just my opinion, i don't think many people do or should. if your cat has an aggravating condition or you've been at this for years and years and years or you're home all the time.....maybe, but 99 out of 100 people.....no :)
