Sooty has diabetes and asthma

I hope I do this right , its my first post. My cat sooty has asthma and diabetes which is controlled with Ventolin and diabetic food. But in the last few months hes developed very itchy skin all his back has scabs and its not fleas as I flea him. Anyone got ideas to help ?
