Skipped AM shot, up to 414, would you give some now?

After a couple of days of lower numbers, a day of not eating, and a rough night, we opted to skip Smokey's shot this morning. Unlike yesterday and last night, she is rising rapidly instead of dropping. Should we give her some insulin now? (next shot is 7 1/2 hours away).

Normally I wouldn't have skipped or done a partial dose because she tends to rise no matter where she's at preshot, but she hasn't been eating nor following normal patterns lately.

Some other info:
She is a high dose kitty - 10.5 U bi.d. - yes she is a high dose kitty, she has a 2cm green mass on her pancreas.
She has had high quantities of blood in her urine for over a month. A course of Orbax lessened the visible signs in the LB, but there was still quite a bit when the vet drew a clean sample. No infection on culture.
We've recently has a mass show on xrays, don't know what is up with it because pre-CT exam found that her heart condition had deteriorated too much.

So, the short of that is Smokey has a lot against her right now and is probably in the end stages of her life. Some of her patterns will likely no longer be relevent. The sudden low numbers could have been a result of not eating as much as she used to, could be a result of a cancer coming on. Our girl that has always be predictable, but not so much anymore.

Should we give her a partial dose now and then a full dose at normal shot time? A half dose now, half dose at normal shot time and then get back to normal in the morning? I hate to leave her climb for almost 8 more hours.
 
Do you work from home and is your shot schedule flexable enough that you could give her AM shot now, and her PM shot 12 hours from now, and then adjust back slowly (in 15 minute increments per shot) to your normal shot schedule?
 
Normally I would say yes because I would just go to work late and work late and my husband does the evening shot. But he is leaving for a week starting tomorrow morning. I'll have draw it up to see what a shedule would look like.

We are more likely going to have to make it up by ajusting amounts in some of the shots that adjusting the time. That is why I was thinking about a reduced shot tonite if we did some now.

I don't know. Levemir, if that's what Smokey is still using, is a depot insulin, and you could have overlapping nadirs if you shoot in the middle of the cycle.

Do you have any R (regular) insulin?

It may not be all that bad. Sometimes, if the cat is way high into the 300s or above, we sometimes will advise that you can shoot an hour early. You would need to be home to monitor if you did that, as an early shot acts like a dose increase. But, one early shot like that would take 2 days off your schedule above.
 
I just tested again and she's still climbing so the bottom line is she needs insulin now so she's not hitting 500s. I'll just have to deal with the schedule to get back on track. Thanks Dyana

eta: Smokey has a very late nadir these days. Seems to be around +8 to +10 and then she's moving back up very quickly after her bottom. Sometimes she even runs a 24 hour cycle instead of a 12, she rarely has a low point in the middle of a 12 hr cycle.
 
I'm glad you decided to go with the shot schedule adjustment rather than a split dose, especially with her recent eating issues. Come on down, nice and easy Smokey. cat_pet_icon My first ever cat when I was a child, was a Smokey, and she looked a lot like your Smokey does.
