Simmie - still running high now, an increase?

Hi Friends,

Simmie is now testing higher consistently though he is now on 0% carb food and treats. I want to increase his latest shot from 2 units to 2.5. I do not have 1/2 units on my syringes (I'm going to get a new script for those this week when I see the vet) so I'll have to eyeball it. .25 would be almost impossible for me to eyeball unfortunately so I would just monitor very closely.

I was going to do a curve tomorrow (at home) but should I first increase his insulin then do the curve on Tuesday?

One big positive step. I gave him a tiny bit of food this morning to take the edge off before testing but not enough to impact his BG levels...per some of our joking and my questions earlier this past week (ie. to take the edge off Simmie I'd need to give him at least a side of beef). Anyway, he ate a little of his snack then kept looking up at me. I had no idea what he was doing. I kept trying to have him eat a little more but he wouldn't and kept looking up at me. I realized that he was expecting the protocol! And he was! I got my test, gave him his shot then let him finishing eating (I know normally it is Test, Feed, Shoot but he was high). Wow!

I guess I am ready to fly the nest and move into one of the insulin groups now. :smile: Love you all!

ps - I know everyone is very busy with our sick little kitty from the last 911. Do not bother replying to this now if you are all very busy.
 
Greetings newbie here myself, so I can't really give advice, but could the higher number be from a bounce? While I am not one to give any advice, I think typically they want to see the AMPS and the PMPS with no food influence.

If you know what Insulin Support Group you will be joining....I would go ahead and post this in that forum, there seem to always be people floating around who are there to help out with this kind of stuff :)

My cat Skooter is on Lantus and we belong to the Tight Regulation support group....we have gotten some great support and information over there.....

In those forums usually your subject line reads, "date CAT NAME AMPS # and then any additional tests you have done throughout the day listed as +#.....

Hi Lydette and super sweet Simmie!

Without going back and looking at all of your posts, it's hard to say, but what I see that's most important is that you need to get some more tests in. We really need to know how low this dose is taking him, and without more mid-cycle testing, we can't say what's going on.

Can you get tests in during the AM cycle somewhere between +4 and +8 (varying so you're not always testing at the same time)? And at night, at least a "before bed" test---Most cats go lower at night, so it's important to get that test in before you go to sleep in case he's going lower so you know if you need to set an alarm to wake up to get another test in.

The Lantus Tight Regulation Forum is the insulin support group you're going to want to come to. Each day, post a new "condo" and let us know how Simmie is doing.

As Molly said, you'd start with the date, cats name and AMPS number..as you get tests in later, you can edit that first subject line and add those results so when we scan the board, we can see how he's responding.

In the body of the message, you'd first post the link to the prior post by Copying the link in the address bar, clicking on the URL tab and then "Pasting" the link...it would look like this Your Link Here[url] Then you give ...ed Lantus users there to help you and Simmie!
 
ARGH - I AM SUCH A JERK!!! I think I screwed up all the good I was doing to Simmie (though he is licking his ice like a champ and seems happy as can be)!

I was going to start one of the regulation forums today and give Simmie a curve but I overslept - my doc put me on a new course of meds and my DH didn't want to wake me, no matter what. Anyway, I ran downstairs like a madwoman , thinking I would just shoot as it was so late but of course I didn't. And he once again, for only the 2nd time, just sat there waiting for me to poke him. Sweet Boy. And surprise, his number was just 256. So as Molly said, he might be in a bounce but how do I know that now?

What do I do? Do I keep him at 11 (which is a problem if we go on vacation next month as the vet can only shoot between 8 am and 8 pm or do I adjust him downward back to his usual start time?

Did I screw him up by waiting so much longer to give him his test food shot (do we have an acronym for this)? :-D

ps - once I know how to proceed I'll move to either the tight or slow regulation forum. I like the idea that this is for the real newbies and emergencies. Gosh you have a great and very logical board here. And it's the people who made it this way.

If you shoot now, can you shoot 12 hours from now?? If not, it's best to just skip it and go back to the schedule that works best for you tonight. You CAN adjust times but only 15 minutes per cycle or 30 minutes per day, so if you have a schedule of 11/11 now and want him on 8/8, you need to start working your way back to that either in 15 minute increments per cycle, or one 30 minute increment per day.

Any time you shoot late, it acts like a dose reduction, so he may have some numbers that don't make much sense for a little while, but no permanent damage done :-D

I strongly suggest when you move to the insulin support group, you go ahead and move to the Tight Regulation forum, even if you want to do the "Relaxed" protocol. There are many more of us on the TR forum and since we're all using Lantus, you'll get the most experienced eyes there to help

The Relaxed Forum is fine, it's just not as busy, so you might have to wait longer to get answers

OK - I'll go to the tight regulation forum. I just worry that I'm still too much of a newbie to be there but I do understand the posting protocol (how to structure the message - thanks all who have helped with that). I do tend to write longer messages (I'm getting better :-) because I love to chat (yes, I did find the Community forum :lol: )

Two of the more practical reason for thinking about the other reg forum is that because of health issues I might not be able to tightly control him as much as I should, with the testing schedule and such. Also, Buddy the Dog has a whole wealth of issues and gets 6x2 meds per days - he's on 12/12 too only not for the big D. :smile:
so sometimes I have to switch my attention over - he has a respiratory issue, bad arthritis and he's going a little weird now...doc says it's an early type of dementia - but he is still very cute - 55 lbs of crazy joy!

Anyway, so what about that? What if I can't really follow the TR recommendations?
 
Don't worry about it if you can't follow the TR protocol.....but actually, to follow it you only need to get 4 tests per day in...the 2 tests before shooting (for safety) and a mid-cycle one during the AM cycle and a "before bed" one on the PM cycle

You can actually squeeze by with just 3 if there's just no way to get a mid-cycle during the day...you just get lots of tests on your days off! We may be able to help you figure out a schedule that would work best for you too if you share what time you have to leave and when you can get home each day. We do have people who work full time jobs and still follow the TR protocol

The big difference between the 2 boards is just activity level more than anything. There are more people on the TR forum than on the relaxed, so if you post there, you may not get an answer for quite awhile if nobody is around. On the TR board, it's much busier so you're more likely to get an answer quickly

If you decide the SLGS (start low, go slow) "relaxed" protocol is the way you want to go, you can still post on the TR forum....you'd just want to put SLGS in your signature line to remind people you're following that protocol (and you'll probably still have to remind them in your condo's...lol)

Ok, I've been convinced. I'll go to the TR tomorrow. I'm planning to do the full curve on Simmie so it's a good time to start.

Chris - thanks for pointing this out.

I just realized, having read this thread, that I joined the wrong Lantus group! :lol: I've been following the Tight Regulation protocol all along (still am as you can see in Alice's spreadsheet).

Don't get me wrong, the "Relaxed" group has been amazing ... Alice wouldn't be doing an OTJ trial run right now without their help and support. I guess as a newbie earlier this month, I just didn't understand the difference!

Congratulations on Alice's OTJ trial too!

It doesn't matter which forum you're on....both have great people willing to help. The TR forum is just "busier" :-D
