Yesterday I put a little bowl of young again zero carb kibble [which Sadie loves] down, thinking I would try the free feed idea because she is dropping weight and ravenous all the time. She seemed fine, ate her dinner and I injected her. At some point after that she had a little loose stool not in the box and she threw up her dinner. Checked her sugar at 4am this morning and it was 252. I figured no more free feed [the kibble wasn't left out overnight anyway] and that the food probably caused it bc I could see the pieces in her vomit. So I fed her usual home cooked breakfast at 4. Went back in at a quarter til 8 and she had vomited that as well--also had pieces of kibble in it. So I gave her zofran and cerenia transdermal and checked her glucose--it is 330.
What should I do? Skip her am insulin? Wait X amount of time and if she hasn't vomited again give it then? Something else?

Did you noticed a hairball in the food she threw up? my Coco is not easy on letting go of hairball, she has very long full hair, and there are times that on any day she has a full meal and right after she throws up all of her food with pieces as well with a giant hairball, it even looks scary:bighug::bighug::cat::cat:

I would not skip insulin for vomiting. Try feeding her again, but small amounts at a time to see if she keep sit down. I'd also try to get some mid cycle tests on her, to make sure she's not going low. Please add the 4am test to the spreadsheet. The spreadsheet is the first thing I open when I read a post.

I'd also check in with the vet. There are a few reasons a cat can be starving all the time and losing weight. It could be unregulated diabetes, hyperthyroidism or a GI condition. A blood work and check up might be a good idea if it's been a while.

How is Sadie doing now?

I skipped it--I am just not knowledgeable enough to feel confident in going ahead with the dose when she was doing something unusual [vomiting], so I erred on the side of caution, or at least to me I did.
Her stomach did settle down so I gave her the pm dose and she is back on track.
She had a pretty extensive workup a month or two ago and thankfully GI and thyroid are okay. She had to be on Clavamox for 4 weeks which ended the end of October. She was in the ER gosh late August/early September and they thought she was in remission and instructed me to stop giving her insulin until I got her in to see an internist at Blue Pearl, but it took a few weeks to get her in bc they are so busy so she was off insulin for a while. They restarted her--she was already dropping weight at that time but they attributed it to the diabetes being unregulated. She was in remission early this year, kind of discouraging that she is in such a mess right now.

do you think I should increase to 1.75?
Not at this time. You have no idea how low the current dose is taking her now. You need to do a curve or some mid cycle tests when she's doing a downward curve to see how low she is going on 1.5 units.
